Indications
- Lower the blood glucose in patients with noninsulin dependent (Type II) diabetes mellitus
- Adjuvant to diet and exercise
- Combination use with metformin
- Combination use with insulin
- Increase the potential for hypoglycemia
Pharmacology
- Sulfonylurea antidiabetic agent
- Decreases blood glucose concentration
- Stimulates insulin release
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
Dosage
- Dependent on desired blood sugar level
- Initial dose 1 mg once daily
- Dose titration based on regular blood sugar monitoring
- Usual dose range in well controlled diabetes 1 to 4 mg daily
- Timing and distribution decided by physician
Administration
- Tablet must be swallowed with sufficient liquid
Interaction
- Potentiates hypoglycemic action with ACE inhibitors
- Potentiates hypoglycemic action with azapropazone
- Attenuates hypoglycemic action with barbiturates
- Masks warning symptoms of hypoglycemic attack with beta-blockers
Contraindications
- Not suitable for insulin dependent diabetes mellitus
- Not suitable for diabetic ketoacidosis
- Not suitable for diabetic coma
- Not suitable for patients with severe hepatic dysfunction
- Not suitable for patients with severe impairment of renal function
- Not suitable for dialysis patients

- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy, changeover to insulin necessary
- Ingestion with breast milk feeding may harm the child, not to be taken by breastfeeding women
Precautions & Warnings
- Initial weeks of treatment may increase risk of hypoglycemia
- Promptly controlled by immediate intake of carbohydrates
Overdose Effects
- Producing hypoglycemia
- Mild hypoglycemic symptoms should be treated aggressively
- Severe hypoglycemic reactions constitute medical emergencies
