Description
- Dactus is a sulfonylurea antidiabetic agent used to lower blood glucose levels in patients with noninsulin dependent (Type II) diabetes mellitus (NIDDM) whose hyperglycaemia cannot be controlled by diet and exercise alone. It is also indicated for use in combination with metformin or insulin when other measures are not sufficient in achieving adequate glycaemic control.

Technical Specifications
- Hypoglycaemic action of a single dose persists for 24 hours
- Initial recommended daily dose of 1 mg
- Usual dose range in patients with well-controlled diabetes is 1 to 4 mg daily
- Should be taken immediately before a substantial breakfast or the first main meal
- Storage temperature should not exceed 30°C

Usability Features

Interactions
- Potentiates hypoglycaemic action with ACE inhibitors, anabolic steroids, fluconazole, etc.
- Attenuates hypoglycaemic action with barbiturates, corticosteroids, diazoxide, etc.
- Concomitant treatment with certain drugs may mask the warning symptoms of hypoglycaemic attack
- Alcohol intake may unpredictably potentiate or attenuate the activity of Dactus
Dosage
- Governed by desired blood sugar level
- Lowest effective dose must be used
- Dose titration should be gradual and based on regular blood sugar monitoring
- Dose adjustment required with changes in weight, lifestyle, or other factors affecting glycaemic control
Administration
- Tablet must be swallowed with sufficient liquid
Indications
- Adjunct to diet and exercise for lowering blood glucose in NIDDM patients
- May be used in combination with metformin or insulin when other measures are inadequate
Contraindications
- Hypersensitivity to Glimepiride, other sulfonylureas, or other sulfonamides
- Severe hepatic dysfunction
- Severe impairment of renal function
- Dialysis patients

Precautions & Warnings
- Initial weeks of treatment pose increased risk of hypoglycaemia
- Immediate intake of carbohydrates to control hypoglycaemia
Overdose Effects
- Can produce hypoglycaemia
- Mild hypoglycaemic symptoms should be treated aggressively with oral glucose and adjustments in drug dosage or meal patterns
- Severe hypoglycaemic reactions with coma or seizure constitute medical emergencies
