Indications
- Adjunct to diet and exercise for lowering blood glucose in Type II diabetes mellitus
- May be used in combination with metformin or insulin for glycaemic control
- May increase potential for hypoglycaemia when used with insulin
- Prescribed when hyperglycaemia cannot be controlled by diet and exercise alone
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates release of insulin from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Hypoglycaemic action persists for 24 hours in non-fasting diabetic patients
Dosage
- Initial dose is 1 mg once daily
- Gradual dose titration based on blood sugar monitoring
- Usual dose range in well controlled diabetes is 1 to 4 mg daily
- Timing and distribution of doses decided by physician
- Sensitivity to insulin increases as control of diabetes improves, may require dose reduction
- No exact dosage relationship with other oral antidiabetics
Administration
- Tablet must be swallowed with sufficient amount of liquid
Interaction
- Potentiates hypoglycaemic action with ACE inhibitors, aminosalicylic acid, anabolic steroids, azapropazone, etc.
- Attenuates hypoglycaemic action with acetazolamide, barbiturates, calcium channel blockers, etc.
- Concomitant treatment with beta-blockers, clonidine, guanethidine or reserpine may mask warning symptoms of hypoglycaemic attack
- Alcohol intake may unpredictably potentiate or attenuate activity of Diaryl
Contraindications
- Not suitable for insulin dependent (Type I) diabetes mellitus or diabetic ketoacidosis
- Must not be used in patients hypersensitive to Glimepiride, other sulfonylureas, other sulfonamides, severe hepatic dysfunction, severe impairment of renal function, and dialysis patients

Pregnancy & Lactation
- Must not be taken during pregnancy, changeover to insulin necessary
- Breastfeeding women must not take Glimepiride, changeover or discontinuation of breastfeeding necessary
Precautions & Warnings
- In the initial weeks of treatment, risk of hypoglycaemia may be increased, necessitates careful monitoring
- Hypoglycaemia can almost be promptly controlled by immediate intake of carbohydrates (glucose or sugar)
Overdose Effects
- Overdosage can produce hypoglycaemia
- Mild hypoglycaemic symptoms should be treated aggressively with oral glucose and adjustments in drug dosage or meal patterns
- Severe hypoglycaemic reactions with coma or seizure are medical emergencies requiring immediate hospitalization
