Therapeutic Class

  • Sulfonylureas

Indications

  • Lowering blood glucose in noninsulin dependent diabetes mellitus (NIDDM)
  • Adjunct to diet and exercise
  • Combination therapy with metformin or insulin
  • Use in conjunction with an oral hypoglycaemic agent

Pharmacology

  • Sulfonylurea antidiabetic agent
  • Stimulates release of insulin from pancreatic beta cells
  • Improves sensitivity of beta cells to glucose stimulus
  • Reduces basal hepatic glucose production
  • Increases peripheral tissue sensitivity to insulin
  • Enhances glucose uptake

Dosage

  • Initial dose of 1 mg once daily
  • Gradual dose titration based on glucose monitoring
  • Usual dose range of 1 to 4 mg daily
  • Timing and distribution of doses decided by physician
  • Swallow with sufficient amount of liquid
  • Dose adjustments based on control of diabetes, weight changes, and lifestyle

Administration

  • Tablet must be swallowed with sufficient liquid

Interaction

  • Potentiating drugs include ACE inhibitors, aminosalicylic acid, anabolic steroids, azapropazone, etc.
  • Attenuating drugs include acetazolamide, barbiturates, calcium channel blockers, etc.
  • Concomitant treatment with beta-receptor blockers, clonidine, guanethidine, or reserpine may mask hypoglycaemic symptoms
  • Alcohol intake may potentiate or attenuate activity in an unpredictable fashion

Contraindications

  • Insulin dependent diabetes mellitus
  • Diabetic ketoacidosis
  • Diabetic coma
  • Hypersensitivity to Glimepiride or other sulfonylureas
  • Severe hepatic dysfunction
  • Severe impairment of renal function
  • Dialysis patients

Side Effects

  • Hypoglycaemia
  • Temporary visual impairment
  • Nausea
  • Vomiting
  • Diarrhoea
  • Abdominal pain
  • Urticaria
  • Fall in blood pressure

Pregnancy & Lactation

  • Not to be taken during pregnancy
  • Changeover to insulin is necessary
  • Harmful when taken during breastfeeding

Precautions & Warnings

  • Increased risk of hypoglycaemia in initial weeks of treatment
  • Necessitates careful monitoring
  • Promptly controlled by immediate intake of carbohydrates (glucose or sugar)

Overdose Effects

  • Can produce hypoglycaemia
  • Mild symptoms treated aggressively with oral glucose and adjustments in drug dosage or meal patterns
  • Severe reactions constitute medical emergencies requiring immediate hospitalization
  • Administer concentrated (50%) glucose solution for hypoglycaemic coma
  • Close monitoring for at least 24 to 48 hours
