Indications
- Adjunct to diet and exercise for lowering blood glucose in noninsulin dependent diabetes mellitus (NIDDM)
- Use in combination with metformin or insulin for adequate glycaemic control
- Potential increase in hypoglycaemia risk with combined use of Dieta and insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates insulin release from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Hypoglycaemic action persists for 24 hours in non-fasting diabetic patients
Dosage
- Determined by desired blood sugar level
- Initial dose usually 1 mg once daily, with gradual increase based on blood sugar monitoring
- Usual dose range in well controlled patients is 1 to 4 mg daily
- Timing and distribution of doses decided by physician considering patient's lifestyle
- Dose adjustment needed with changes in weight or lifestyle
- Specific guidelines for changeover from other oral antidiabetics to Glimepiride
Administration
- Tablet must be swallowed with sufficient amount of liquid
Interaction
- Potentiating drugs include ACE inhibitors, chloramphenicol, fluoxetine, probenecid, sulphonpyrazone, tetracyclines, and more
- Attenuating drugs include barbiturates, corticosteroids, diuretics, nicotinic acid, phenothiazines, thyroid hormones, and more
- Possible masking of hypoglycaemic attack symptoms with certain concomitant treatments
- Impact of alcohol intake on the activity of Dieta
Contraindications
- Not suitable for insulin dependent diabetes mellitus or diabetic coma
- Must not be used in patients hypersensitive to Glimepiride, other sulfonylureas, other sulfonamides, severe hepatic dysfunction, severe impairment of renal function, and dialysis patients

Pregnancy & Lactation
- Not to be taken during pregnancy, requiring changeover to insulin
- Ingestion with breastfeeding may harm the child, requiring changeover or discontinuation of breastfeeding
Precautions & Warnings
- Increased risk of hypoglycaemia in initial weeks of treatment
- Immediate intake of carbohydrates to control hypoglycaemia
Overdose Effects
- Potential to produce hypoglycaemia
- Treatment of mild hypoglycaemic symptoms with oral glucose and adjustment in dosage or meal patterns
- Severe hypoglycaemic reactions constitute medical emergencies requiring immediate hospitalization
