Medicine Details

Indications

  • Adjunct to diet and exercise for Type II diabetes mellitus
  • May be used with metformin for glycaemic control
  • May be used in combination with insulin
  • Increases potential for hypoglycaemia when combined with insulin

Pharmacology

  • Sulfonylurea antidiabetic agent
  • Stimulates insulin release from pancreatic beta cells
  • Improves sensitivity of beta cells to glucose stimulus
  • Reduces basal hepatic glucose production
  • Increases peripheral tissue sensitivity to insulin

Dosage

  • Determined by desired blood sugar level
  • Lowest effective dose for desired metabolic control
  • Initial dose is 1 mg once daily
  • Daily dose can be increased gradually
  • Usual dose range is 1 to 4 mg daily
  • Timing and distribution decided by physician based on patient's lifestyle
  • Dose adjustment required with changes in patient's weight or lifestyle
  • No exact dosage relationship with other blood sugar lowering agents

Administration

  • Tablet must be swallowed with sufficient liquid

Interaction

  • Potentiates hypoglycaemic action with various drugs
  • Attenuates hypoglycaemic action with certain drugs
  • May mask warning symptoms of hypoglycaemic attack
  • Alcohol intake may affect the activity of the medication

Contraindications

  • Not suitable for insulin dependent diabetes mellitus
  • Not suitable for diabetic ketoacidosis or coma
  • Hypersensitivity to Glimepiride or sulfonylureas
  • Severe hepatic dysfunction
  • Severe impairment of renal function
  • Not for dialysis patients

  • Abdominal pain
  • Urticaria
  • Fall in blood pressure

Pregnancy & Lactation

  • Not to be taken during pregnancy
  • Changeover to insulin necessary
  • Harmful if taken during breastfeeding
  • Changeover to insulin or discontinuation of breastfeeding necessary

Precautions & Warnings

  • Risk of increased hypoglycaemia in initial weeks of treatment
  • Dosage adjustment needed if hypoglycaemia risk present
  • Prompt control of hypoglycaemia with immediate intake of carbohydrates

Overdose Effects

  • Overdosage can produce hypoglycaemia
  • Mild symptoms treated with oral glucose and adjustments in drug dosage or meal patterns
  • Severe reactions require immediate hospitalization and IV glucose injection
  • Close monitoring needed for 24 to 48 hours
