Indications
- Adjunct to diet and exercise for noninsulin dependent diabetes mellitus
- May be used concomitantly with metformin
- Indicated for use in combination with insulin
- May increase potential for hypoglycemia
Pharmacology
- Sulfonylurea antidiabetic agent
- Decreases blood glucose concentration
- Stimulates release of insulin from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
Dosage
- Initial dose is 1 mg once daily
- Gradual dose titration based on regular blood sugar monitoring
- Usual dose range in patients with well controlled diabetes is 1 to 4 mg daily
- Single daily dose usually taken before substantial breakfast or first main meal
Administration
- Tablet must be swallowed with sufficient amount of liquid
Interaction
- Potentiating drugs include ACE inhibitors, aminosalicylic acid, anabolic steroids, etc.
- Attenuating drugs include barbiturates, corticosteroids, diazoxide, etc.
- Certain drugs may lead to potentiation or weakening of blood-glucose-lowering effect
Contraindications
- Not suitable for insulin dependent diabetes mellitus
- Not for diabetic ketoacidosis or diabetic coma
- Must not be used in patients hypersensitive to Glimepiride or sulfonylureas
- Not for patients with severe hepatic or renal dysfunction or dialysis patients

- Hypoglycemia
- Temporary visual impairment
- Nausea
- Vomiting
- Diarrhea
- Abdominal pain
- Urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Must not be taken during pregnancy
- Changeover to insulin is necessary
- Ingestion with breast milk feeding may harm the child
- Not for breastfeeding women
Precautions & Warnings
- Initial weeks of treatment may increase risk of hypoglycemia
- Necessitates careful monitoring
- Promptly controlled by immediate intake of carbohydrates
Overdose Effects
- Overdosage can produce hypoglycemia
- Mild symptoms should be treated aggressively with oral glucose
- Severe reactions with coma require immediate hospitalization
- Close monitoring for at least 24 to 48 hours
