Indications
- Adjunct to diet and exercise for lowering blood glucose in noninsulin dependent diabetes mellitus (NIDDM)
- Concomitant use with metformin
- Use in combination with insulin for lowering blood glucose in conjunction with an oral hypoglycemic agent
- Increased potential for hypoglycemia with combined use of Glimirid and insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Decreases blood glucose concentration
- Stimulates the release of insulin from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin and glucose uptake
- Hypoglycemic action lasting 24 hours
Dosage
- Dependent on desired blood sugar level
- Initial dose of 1 mg once daily
- Gradual dose titration based on regular blood sugar monitoring
- Usual dose range for patients with well controlled diabetes is 1 to 4 mg daily
- Timing and distribution of doses decided by physician in consideration of patient's lifestyle
- Secondary dosage adjustment as control of diabetes improves
- Changeover from other oral antidiabetics to Glimepiride
Administration
- Tablet form
- Must be swallowed with sufficient amount of liquid
Interaction
- Potentiate hypoglycemic action: ACE inhibitors, aminosalicylic acid, anabolic steroids, azapropazone, chloramphenicol, and more
- Attenuate hypoglycemic action: Acetazolamide, barbiturates, calcium channel blockers, corticosteroids, and more
- Masking of hypoglycemic attack symptoms with beta-receptor blocker, clonidine, guanethidine or reserpine
- Unpredictable effects with acute and chronic alcohol intake
Contraindications
- Not suitable for insulin dependent diabetes mellitus
- Not for treatment of diabetic ketoacidosis or diabetic coma
- Hypersensitivity to Glimepiride, sulfonylureas, sulfonamides
- Severe hepatic dysfunction, severe impairment of renal function, dialysis patients

Pregnancy & Lactation
- Not to be taken during pregnancy; changeover to insulin necessary

Precautions & Warnings
- Risk of hypoglycemia in initial weeks
- Careful monitoring and dosage adjustment to control hypoglycemia
Overdose Effects
- Can produce hypoglycemia
- Mild hypoglycemic symptoms without loss of consciousness or neurologic findings treated aggressively with oral glucose and adjustments in drug dosage or meal patterns
- Severe hypoglycemic reactions with coma, seizure, or other neurological impairment constitute medical emergencies requiring immediate hospitalization
