Indications
- Adjunct to diet and exercise for lowering blood glucose in non-insulin dependent diabetes mellitus (NIDDM)
- Combination use with metformin
- Combination use with insulin for lowering blood glucose
- Increased risk for hypoglycemia with combined use of Glimirid and insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Decreases blood glucose concentration
- Stimulates insulin release from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
Dosage
- Dosage governed by desired blood sugar level
- Initial dose usually 1 mg once daily, with gradual dose increases based on glucose monitoring
- Usual dose range for well-controlled diabetes is 1 to 4 mg daily
- Timing and distribution of doses decided by physician based on patient's lifestyle
- Dose adjustment required with improved diabetes control
Administration
- Tablet must be swallowed with sufficient liquid
Interaction
- Drugs potentiating hypoglycemic action of Glimirid
- Drugs attenuating hypoglycemic action of Glimirid
- Potential for masking hypoglycemic attack symptoms with concomitant beta-receptor blocker, clonidine, guanethidine, or reserpine use
- Variable effect of alcohol intake on Glimirid activity
Contraindications
- Not suitable for insulin dependent diabetes mellitus
- Not suitable for diabetic ketoacidosis or diabetic coma
- Hypersensitivity to Glimirid, other sulfonylureas, or sulfonamides
- Severe hepatic dysfunction
- Severe impairment of renal function
- Not for use in dialysis patients

Pregnancy & Lactation
- Not to be taken during pregnancy

Precautions & Warnings
- Increased risk of hypoglycemia in initial weeks of treatment
- Necessity for careful monitoring and dosage adjustment in case of hypoglycemia
- Prompt control of hypoglycemia with immediate intake of carbohydrates
Overdose Effects
- Hypoglycemia in cases of overdosage
- Mild hypoglycemic symptoms treated with oral glucose and adjustments in drug dosage or meal patterns
- Severe hypoglycemic reactions constituting medical emergencies
