Indications
- Adjunct to diet and exercise to lower blood glucose in noninsulin dependent diabetes mellitus
- Used in combination with insulin to lower blood glucose
- May be used concomitantly with metformin
Pharmacology
- Decreases blood glucose concentration
- Stimulates the release of insulin from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Hypoglycemic action persists for 24 hours in non-fasting diabetic patients
Dosage
- Based on desired blood sugar level
- Initial dose is 1 mg once daily
- Usual dose range is 1 to 4 mg daily
- Timing and distribution of doses decided by physician
- Dose adjustment as control of diabetes improves
- Initial daily dose is 1 mg when switching from other oral blood sugar lowering agents
Administration
Tablet must be swallowed with sufficient liquid
Interaction
- Potentiates hypoglycemic action:
- ACE inhibitors
- Glucagon
- MAO-inhibitors
- Sulphonpyrazone
- Attenuates hypoglycemic action:
- Barbiturates
- Calcium channel blockers
- Corticosteroids
Contraindications
- Insulin dependent diabetes mellitus
- Diabetic ketoacidosis
- Diabetic coma
- Hypersensitivity to Glimepiride or sulfonylureas
- Severe hepatic dysfunction
- Severe impairment of renal function
- Dialysis patients
Side Effects
- Hypoglycemia
- Temporary visual impairment
- Nausea
- Vomiting
- Diarrhea
- Abdominal pain
- Urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy
- Changeover to insulin necessary
- Not to be taken by breastfeeding women
Precautions & Warnings
- Increased risk of hypoglycemia in initial weeks of treatment
- Monitoring required
- Immediate intake of carbohydrates for hypoglycemia
Overdose Effects
- Can produce hypoglycemia
- Treat aggressively with oral glucose for mild symptoms
- Hospitalization for severe hypoglycemic reactions
