Indications
- Adjunct to diet and exercise for lowering blood glucose in patients with Type II diabetes mellitus (NIDDM)
- Use in combination with metformin or insulin for adequate glycaemic control
- Increased potential for hypoglycaemia in combined use with insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates insulin release from pancreatic beta cells
- Improves beta cells' sensitivity to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
Dosage
- Dosage based on desired blood sugar level
- Initial dose of 1 mg once daily
- Gradual dose titration based on blood sugar monitoring
- Dose range of 1 to 4 mg daily in well controlled diabetes
- Single daily dose taken before breakfast or first main meal
Administration
- Tablet must be swallowed with sufficient amount of liquid
Interaction
- Potentiating interactions with ACE inhibitors, aminosalicylic acid, chloramphenicol, fluoxetine, sulphonpyrazone, etc.
- Attenuating interactions with barbiturates, corticosteroids, diuretics, nicotinic acid, etc.
- Masking of hypoglycaemic attack symptoms by beta-receptor blockers, clonidine, etc.
Contraindications
- Not suitable for insulin dependent diabetes mellitus or diabetic ketoacidosis
- Hypersensitivity to Glimepiride, sulfonylureas, sulfonamides
- Severe hepatic dysfunction, severe impairment of renal function, dialysis patients
Side Effects
- Hypoglycaemia
- Temporary visual impairment
- Nausea, vomiting, diarrhoea, abdominal pain
- Urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy, changeover to insulin necessary
- Harmful for breastfeeding, changeover or discontinuation necessary
Precautions & Warnings
- Increased risk of hypoglycaemia in initial treatment weeks
- Necessitates careful monitoring and possible dosage adjustment
- Prompt control of hypoglycaemia by immediate intake of carbohydrates
Overdose Effects
- Hypoglycaemia from overdosage
- Treatment with oral glucose for mild symptoms
- Immediate hospitalization for severe hypoglycaemic reactions
