Indications
- Adjunct to diet and exercise for lowering blood glucose in Type II diabetes
- Use in combination with metformin for adequate glycaemic control
- Can be used in combination with insulin for lowering blood glucose
- Potential for hypoglycaemia when combined with insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates insulin release from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Hypoglycaemic action lasts for 24 hours
Dosage
- Initial dose of 1 mg once daily
- Dose titration based on blood sugar monitoring
- Usual dose range for well-controlled diabetes is 1 to 4 mg daily
- Single daily dose before breakfast
- Dosage adjustments based on changes in sensitivity to insulin
Administration
- Tablet must be swallowed with sufficient liquid
Interactions
- Interactions with various drugs that potentiate or attenuate hypoglycaemic action
- Caution needed with ACE inhibitors, aminosalicylic acid, corticosteroids
- Drugs like barbiturates, diuretics, and beta-blockers may weaken hypoglycaemic effects
Contraindications
- Not suitable for insulin-dependent diabetes mellitus
- Contraindicated in diabetic ketoacidosis
- Hypersensitivity to Glimepiride or sulfonylureas
- Avoid in severe hepatic dysfunction and renal impairment
Side Effects
- Hypoglycaemia
- Temporary visual impairment
- Nausea, vomiting, diarrhea
- Abdominal pain, urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Avoid during pregnancy, switch to insulin
- Not safe for breastfeeding women
- Discontinue breastfeeding or switch over
Precautions & Warnings
- Increased risk of hypoglycaemia in initial treatment
- Dosage adjustments may be necessary
- Prompt control of hypoglycaemia with carbohydrate intake
Overdose Effects
- Hypoglycaemia from overdosage can occur
- Mild symptoms can be treated with oral glucose
- Severe reactions require immediate hospitalization
