Indications

  • Lower blood glucose in noninsulin dependent diabetes mellitus (NIDDM)
  • Adjunct to diet and exercise for NIDDM
  • Combination with metformin for glycaemic control
  • Combination with insulin for glycaemic control

Pharmacology

  • Stimulates insulin release from pancreatic beta cells
  • Improves beta cells sensitivity to glucose
  • Reduces basal hepatic glucose production
  • Increases peripheral tissue sensitivity to insulin

Dosage

  • Initial dose is 1 mg once daily
  • Dose titration based on glucose monitoring
  • Usual dose range for well-controlled diabetes is 1 to 4 mg daily

Administration

  • Tablet must be swallowed with sufficient liquid

Interaction

  • Potentiates: ACE inhibitors, sulphonamides, quinolones
  • Attenuates: Calcium channel blockers, corticosteroids, diuretics

Contraindications

  • Insulin dependent diabetes mellitus
  • Diabetic ketoacidosis
  • Diabetic coma
  • Hypersensitivity to sulfonylureas or sulfonamides
  • Severe hepatic dysfunction
  • Severe impairment of renal function
  • Dialysis patients

  • Hypoglycaemia
  • Temporary visual impairment
  • Nausea
  • Vomiting
  • Diarrhoea
  • Abdominal pain
  • Urticaria
  • Fall in blood pressure

Pregnancy & Lactation

  • Not to be taken during pregnancy; changeover to insulin necessary
  • Not to be taken by breastfeeding women

Precautions & Warnings

  • Increased risk of hypoglycaemia in initial weeks of treatment
  • Hypoglycaemia can be controlled by immediate intake of carbohydrates

Overdose Effects

  • Can produce hypoglycaemia
  • Mild symptoms should be treated with oral glucose and drug dosage adjustments
