Indications
- Lower blood glucose in noninsulin dependent diabetes mellitus (NIDDM)
- Adjunct to diet and exercise for NIDDM
- Combination with metformin for glycaemic control
- Combination with insulin for glycaemic control
Pharmacology
- Stimulates insulin release from pancreatic beta cells
- Improves beta cells sensitivity to glucose
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
Dosage
- Initial dose is 1 mg once daily
- Dose titration based on glucose monitoring
- Usual dose range for well-controlled diabetes is 1 to 4 mg daily
Administration
- Tablet must be swallowed with sufficient liquid
Interaction
- Potentiates: ACE inhibitors, sulphonamides, quinolones
- Attenuates: Calcium channel blockers, corticosteroids, diuretics
Contraindications
- Insulin dependent diabetes mellitus
- Diabetic ketoacidosis
- Diabetic coma
- Hypersensitivity to sulfonylureas or sulfonamides
- Severe hepatic dysfunction
- Severe impairment of renal function
- Dialysis patients

- Diarrhoea
- Abdominal pain
- Urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy; changeover to insulin necessary
- Not to be taken by breastfeeding women
Precautions & Warnings
- Increased risk of hypoglycaemia in initial weeks of treatment
- Hypoglycaemia can be controlled by immediate intake of carbohydrates
Overdose Effects
- Can produce hypoglycaemia
- Mild symptoms should be treated with oral glucose and drug dosage adjustments
