Therapeutic Class

  • Sulfonylureas

Indications

  • Adjunct to diet and exercise to lower blood glucose in noninsulin dependent (Type II) diabetes mellitus (NIDDM) patients
  • May be used in combination with metformin when other treatments are inadequate
  • Indicated for use in combination with insulin in certain cases

Pharmacology

  • Primary mechanism of action is stimulating insulin release from pancreatic beta cells
  • Improves sensitivity of beta cells to glucose stimulus
  • Reduces basal hepatic glucose production
  • Increases peripheral tissue sensitivity to insulin and glucose uptake
  • Hypoglycaemic action persists for 24 hours in non-fasting diabetic patients

Dosage

  • Initial dose usually 1 mg once daily
  • Dose titration based on regular blood glucose monitoring
  • Usual dose range in well controlled diabetes is 1 to 4 mg daily
  • Timing and distribution of doses decided by physician

Administration

  • Tablet must be swallowed with sufficient liquid

Interaction

  • May be potentiated by various drugs including ACE inhibitors, aminosalicylic acid, fluoxetine, etc.
  • May be attenuated by drugs like barbiturates, corticosteroids, diazoxide, etc.
  • Concomitant treatment with certain agents may mask hypoglycaemic symptoms

Contraindications

  • Not suitable for insulin dependent (type I) diabetes mellitus
  • Not for treatment of diabetic ketoacidosis or diabetic coma
  • Must not be used in patients hypersensitive to glimepiride or other sulfonylureas
  • Not for patients with severe hepatic or renal dysfunction, or dialysis patients

Side Effects

  • Hypoglycaemia
  • Temporary visual impairment
  • Nausea, vomiting, diarrhea, abdominal pain
  • Urticaria, fall in blood pressure

Pregnancy & Lactation

  • Must not be taken during pregnancy; insulin changeover necessary
  • Ingestion with breast milk feeding may harm the child

Precautions & Warnings

  • Initial weeks of treatment may pose an increased risk of hypoglycaemia
  • Hypoglycaemia can be promptly controlled by immediate intake of carbohydrates

Overdose Effects

  • Overdosage can produce hypoglycaemia
  • Mild symptoms should be treated aggressively with oral glucose and adjustments in drug dosage or meal patterns
  • Severe reactions require immediate hospitalization and IV glucose

  • Keep away from light and out of the reach of children
