Indications
- Adjunct to diet and exercise to lower blood glucose in noninsulin dependent (Type II) diabetes mellitus (NIDDM) patients
- May be used in combination with metformin when other treatments are inadequate
- Indicated for use in combination with insulin in certain cases
Pharmacology
- Primary mechanism of action is stimulating insulin release from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin and glucose uptake
- Hypoglycaemic action persists for 24 hours in non-fasting diabetic patients
Dosage
- Initial dose usually 1 mg once daily
- Dose titration based on regular blood glucose monitoring
- Usual dose range in well controlled diabetes is 1 to 4 mg daily
- Timing and distribution of doses decided by physician
Administration
- Tablet must be swallowed with sufficient liquid
Interaction
- May be potentiated by various drugs including ACE inhibitors, aminosalicylic acid, fluoxetine, etc.
- May be attenuated by drugs like barbiturates, corticosteroids, diazoxide, etc.
- Concomitant treatment with certain agents may mask hypoglycaemic symptoms
Contraindications
- Not suitable for insulin dependent (type I) diabetes mellitus
- Not for treatment of diabetic ketoacidosis or diabetic coma
- Must not be used in patients hypersensitive to glimepiride or other sulfonylureas
- Not for patients with severe hepatic or renal dysfunction, or dialysis patients
Side Effects
- Hypoglycaemia
- Temporary visual impairment
- Nausea, vomiting, diarrhea, abdominal pain
- Urticaria, fall in blood pressure
Pregnancy & Lactation
- Must not be taken during pregnancy; insulin changeover necessary
- Ingestion with breast milk feeding may harm the child
Precautions & Warnings
- Initial weeks of treatment may pose an increased risk of hypoglycaemia
- Hypoglycaemia can be promptly controlled by immediate intake of carbohydrates
Overdose Effects
- Overdosage can produce hypoglycaemia
- Mild symptoms should be treated aggressively with oral glucose and adjustments in drug dosage or meal patterns
- Severe reactions require immediate hospitalization and IV glucose
