Indications

  • Adjunct to diet and exercise to lower blood glucose in noninsulin dependent diabetes mellitus (NIDDM)
  • May be used concomitantly with metformin
  • Indicated for use in combination with insulin
  • Potential for hypoglycaemia with combined use of insulin and glimepiride

Pharmacology

  • Decreases blood glucose concentration
  • Stimulates insulin release from pancreatic beta cells
  • Improves sensitivity of beta cells to glucose stimulus
  • Reduces basal hepatic glucose production
  • Increases peripheral tissue sensitivity to insulin
  • Hypoglycaemic action persists for 24 hours in non-fasting diabetic patients

Dosage

  • Initial dose is 1 mg once daily
  • Dose titration based on regular blood sugar monitoring
  • Usual dose range in well controlled diabetes is 1 to 4 mg daily
  • Timing and distribution of doses decided by physician
  • Dose adjustment with change in patient's weight or lifestyle

Administration

Tablet must be swallowed with sufficient amount of liquid

  • Potentiate hypoglycaemic action:
    • ACE inhibitors
    • Aminosalicylic acid
    • Anabolic steroids
    • Azapropazone
    • Chloramphenicol
    • Coumarin derivatives
  • Attenuate hypoglycaemic action:
    • Acetazolamide
    • Barbiturates
    • Calcium channel blockers
    • Corticosteroids
    • Diazoxide

Contraindications

  • Not suitable for insulin dependent diabetes mellitus
  • Not for treatment of diabetic ketoacidosis or coma
  • Not for patients hypersensitive to glimepiride or other sulfonylureas
  • Not for patients with severe hepatic or renal dysfunction
  • Not for dialysis patients

  • Temporary visual impairment
  • Nausea
  • Vomiting
  • Diarrhoea
  • Abdominal pain
  • Urticaria
  • Fall in blood pressure

Pregnancy & Lactation

  • Not to be taken during pregnancy
  • Changeover to insulin necessary
  • Not to be taken by breastfeeding women

Precautions & Warnings

  • Increased risk of hypoglycaemia in initial weeks of treatment
  • Hypoglycaemia control with immediate intake of carbohydrates

Overdose Effects

  • Hypoglycaemia
  • Treatment with oral glucose and adjustments in drug dosage or meal patterns
  • Severe cases require immediate hospitalization
