Indications
- Adjunct to diet and exercise to lower blood glucose in noninsulin dependent diabetes mellitus (NIDDM)
- May be used concomitantly with metformin
- Indicated for use in combination with insulin
- Potential for hypoglycaemia with combined use of insulin and glimepiride
Pharmacology
- Decreases blood glucose concentration
- Stimulates insulin release from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Hypoglycaemic action persists for 24 hours in non-fasting diabetic patients
Dosage
- Initial dose is 1 mg once daily
- Dose titration based on regular blood sugar monitoring
- Usual dose range in well controlled diabetes is 1 to 4 mg daily
- Timing and distribution of doses decided by physician
- Dose adjustment with change in patient's weight or lifestyle
Administration
Tablet must be swallowed with sufficient amount of liquid

Contraindications
- Not suitable for insulin dependent diabetes mellitus
- Not for treatment of diabetic ketoacidosis or coma
- Not for patients hypersensitive to glimepiride or other sulfonylureas
- Not for patients with severe hepatic or renal dysfunction
- Not for dialysis patients

- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy
- Changeover to insulin necessary
- Not to be taken by breastfeeding women
Precautions & Warnings
- Increased risk of hypoglycaemia in initial weeks of treatment
- Hypoglycaemia control with immediate intake of carbohydrates
Overdose Effects
- Hypoglycaemia
- Treatment with oral glucose and adjustments in drug dosage or meal patterns
- Severe cases require immediate hospitalization
