Indications
- Adjunct to diet and exercise for lowering blood glucose in patients with noninsulin dependent (Type II) diabetes mellitus
- Use in combination with metformin or insulin for glycaemic control
- Potential for hypoglycaemia when used in combination with insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates insulin release from pancreatic beta cells
- Improves beta cell sensitivity to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
Dosage
- Initial dose is 1 mg once daily
- Gradual dose titration based on regular blood sugar monitoring
- Usual dose range for well-controlled diabetes is 1 to 4 mg daily
- Timing and distribution of doses decided by physician
Administration
- Tablet must be swallowed with sufficient liquid
Interaction
- Potentiate hypoglycaemic action: ACE inhibitors, corticosteroids, MAO-inhibitors, sulfonamide antibiotics
- Attenuate hypoglycaemic action: Barbiturates, calcium channel blockers, glucagon, nicotinic acid
Contraindications
- Not suitable for insulin dependent (type I) diabetes mellitus
- Not suitable for diabetic ketoacidosis or diabetic coma
- Not suitable for patients hypersensitive to Glimepiride or other sulfonylureas
- Not suitable for patients with severe hepatic dysfunction or renal impairment

- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy; changeover to insulin necessary
- Not to be taken by breastfeeding women
Precautions & Warnings
- Initial weeks may have increased risk of hypoglycaemia
- Necessitates careful monitoring and dosage adjustment
- Prompt control of hypoglycaemia with immediate intake of carbohydrates
Overdose Effects
- Can produce hypoglycaemia
- Mild symptoms treated aggressively with oral glucose
- Severe reactions with coma or seizure are medical emergencies
