Indications
- Adjunct to diet and exercise to lower blood glucose in noninsulin dependent (Type II) diabetes mellitus (NIDDM)
- Use in combination with metformin when diet, exercise, and Limpet or metformin alone do not result in adequate glycaemic control
- Use in combination with insulin to lower blood glucose in patients whose hyperglycaemia cannot be controlled by diet and exercise
- Potential for increased susceptibility to hypoglycaemia with combined use of Limpet and insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates release of insulin from functioning pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin and glucose uptake
- Hypoglycaemic action persists for 24 hours in non-fasting diabetic patients
Dosage
- Initial dose of 1 mg once daily
- Gradual titration based on regular blood sugar monitoring
- Maintenance dose range of 1 to 4 mg daily
- Timing and distribution of doses decided by physician based on patient's current lifestyle
- Importance of not skipping meals after taking the drug
- Consideration for dose adjustment with changes in patient's weight or lifestyle
Administration
- Tablet form
- To be swallowed with sufficient amount of liquid
Interaction
- Potentiating drugs: ACE inhibitors, aminosalicylic acid, anabolic steroids, azapropazone, etc.
- Attenuating drugs: Acetazolamide, barbiturates, calcium channel blockers, corticosteroids, etc.
- Masking of hypoglycaemic attack symptoms by concomitant treatment with certain drugs
- Unpredictable effects with acute and chronic alcohol intake
Contraindications
- Not suitable for insulin dependent (type I) diabetes mellitus
- Not for treatment of diabetic ketoacidosis or diabetic coma
- Hypersensitivity to Glimepiride, other sulfonylureas, other sulfonamides
- Severe hepatic dysfunction, severe impairment of renal function, dialysis patients
Side Effects
- Hypoglycaemia
- Temporary visual impairment
- Nausea, vomiting, diarrhoea, abdominal pain
- Urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy; changeover to insulin necessary
- Harmful effects with breast milk feeding; not to be taken by breastfeeding women
Precautions & Warnings
- Increased risk of hypoglycaemia in initial weeks of treatment
- Necessity for careful monitoring and potential dosage adjustment
- Prompt control of hypoglycaemia by immediate intake of carbohydrates (glucose or sugar)
Overdose Effects
- Hypoglycaemia with mild or severe symptoms
- Treatment with oral glucose and adjustments in drug dosage or meal patterns
- Severe reactions constitute medical emergencies requiring hospitalization
- Continuous monitoring for at least 24 to 48 hours after hypoglycaemic coma
