Indications
- Adjunct to diet and exercise to lower blood glucose in noninsulin dependent diabetes mellitus (NIDDM)
- Use in combination with metformin or insulin for glycaemic control
- May increase potential for hypoglycaemia
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates insulin release from pancreatic beta cells
- Improves beta cells sensitivity to glucose
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
Dosage
- Governed by desired blood sugar level
- Initial dose is 1 mg once daily
- May be increased based on blood sugar monitoring
- Usual dose range is 1 to 4 mg daily
- Timing and distribution decided by physician
- Dose adjustment with changes in weight or lifestyle
Administration
Tablets must be swallowed with sufficient liquid

- Potentiates hypoglycaemic action with:
- ACE inhibitors
- Cyclophosphamide
- Fluconazole
- Miconazole
- Tetracyclines
- Beta-blockers
- Attenuates hypoglycaemic action with:
- Barbiturates

- Mask warning symptoms of hypoglycaemic attack:

Contraindications
- Insulin dependent diabetes mellitus
- Diabetic ketoacidosis
- Severe hepatic dysfunction
- Severe impairment of renal function
- Hypersensitivity to sulfonylureas
- Dialysis patients

- Nausea

- Diarrhoea
- Abdominal pain
- Urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Must not be taken during pregnancy
- Changeover to insulin necessary
- Harmful when breastfeeding
- Discontinuation of breastfeeding necessary
Precautions & Warnings
- Initial weeks may increase risk of hypoglycaemia
- Careful monitoring required
- Promptly controlled by immediate intake of carbohydrates
Overdose Effects
- Can produce hypoglycaemia
- Treat with oral glucose and dosage adjustments
- Severe reactions constitute medical emergencies
- Require immediate hospitalization
