Indications
- Adjunct to diet and exercise for noninsulin dependent diabetes mellitus (Type II)
- May be used with metformin if diet, exercise, and monotherapy are inadequate
- Use in combination with insulin for uncontrolled hyperglycaemia
- Potential for hypoglycaemia with combined use of Losucon and insulin
Pharmacology
- Decreases blood glucose concentration
- Stimulates insulin release from pancreatic beta cells
- Improves beta cells' sensitivity to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Enhances glucose uptake
Dosage
- Initial dose of 1 mg once daily
- Increase in dose based on regular blood sugar monitoring
- Usual dose range for well controlled diabetes is 1 to 4 mg daily
- Single daily dose taken immediately before a substantial breakfast or first main meal
- Dose adjustment based on improved diabetes control and changes in weight or lifestyle
Administration
- Tablet must be swallowed with sufficient liquid
Interaction
- Potentiating drugs include ACE inhibitors, anabolic steroids, coumarin derivatives, fluconazole, quinolones, etc.
- Attenuating drugs include barbiturates, corticosteroids, diuretics, glucagon, phenothiazines, etc.
- Potential masking of hypoglycaemic symptoms by beta-receptor blockers and other drugs
Contraindications
- Not suitable for insulin dependent diabetes mellitus or diabetic ketoacidosis
- Hypersensitivity to Glimepiride, sulfonylureas, or sulfonamides
- Severe hepatic dysfunction, severe impairment of renal function, and dialysis patients

Pregnancy & Lactation
- Must not be taken during pregnancy
- Patients planning pregnancy should switch to insulin
- Not to be taken by breastfeeding women
Precautions & Warnings
- Increased risk of hypoglycaemia in initial weeks of treatment
- Necessitates careful monitoring
- Prompt control of hypoglycaemia by immediate intake of carbohydrates (glucose or sugar)
Overdose Effects
- Hypoglycaemic symptoms without loss of consciousness treated with oral glucose and dosage adjustments
- Severe reactions constitute medical emergencies requiring immediate hospitalization
- Rapid intravenous injection of concentrated glucose solution in case of hypoglycaemic coma
