Indications

  • Adjunct to diet and exercise for lowering blood glucose in noninsulin dependent (Type II) diabetes mellitus (NIDDM) patients
  • Can be used in combination with metformin or insulin for better glycaemic control

Pharmacology

  • Decreases blood glucose concentration
  • Stimulates insulin release from pancreatic beta cells

Dosage

  • Initial dose is 1 mg once daily
  • Usual dose range is 1 to 4 mg daily
  • Dosing based on regular blood sugar monitoring
  • Distribution of doses depends on the patient's lifestyle

Administration

  • Tablet must be swallowed with sufficient liquid

Interaction

  • Potentiating interactions with various drugs such as ACE inhibitors, fluoxetine, quinolones
  • Attenuating interactions with drugs like barbiturates, corticosteroids, nicotinic acid

Contraindications

  • Not suitable for insulin dependent (type I) diabetes mellitus
  • Hypersensitivity to Glimepiride or sulfonamides
  • Severe hepatic dysfunction and impairment of renal function

Side Effects

  • Hypoglycaemia
  • Temporary visual impairment
  • Nausea, vomiting, diarrhoea, abdominal pain
  • Urticaria
  • Fall in blood pressure

Pregnancy & Lactation

  • Not to be taken during pregnancy
  • Changeover to insulin necessary
  • Harmful if taken during breastfeeding

Precautions & Warnings

  • Increased risk of hypoglycaemia in initial weeks of treatment
  • Prompt control of hypoglycaemia by immediate intake of carbohydrates

Overdose Effects

  • Can produce hypoglycaemia
  • Mild symptoms can be treated with oral glucose and dosage adjustments
  • Severe reactions constitute medical emergencies requiring hospitalization

  • Do not store above 30°C
  • Keep away from light and out of the reach of children
