Indications
- Adjunct to diet and exercise for lowering blood glucose in Type II diabetes mellitus
- Use in combination with insulin for lowering blood glucose
- Potential for hypoglycaemia when combined with insulin
- Adjunct to diet and exercise when metformin alone does not result in adequate glycaemic control
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates insulin release from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Effect on non-fasting diabetic patients
Dosage
- Governed by desired blood sugar level
- Initial dose of 1 mg once daily
- Dose titration based on regular blood sugar monitoring
- Usual dose range in well controlled diabetes is 1 to 4 mg daily
- Timing and distribution of doses decided by physician
- Secondary dosage adjustment as control of diabetes improves
- Changeover from other oral antidiabetics to Glimepiride
Administration
- Tablet must be swallowed with sufficient amount of liquid
Interaction
- Drugs that potentiate hypoglycaemic action of Glimepiride
- Drugs that attenuate hypoglycaemic action of Glimepiride
- Effects of coadministration with beta-blockers, clonidine, guanethidine, and reserpine
- Effects of acute and chronic alcohol intake
Contraindications
- Not suitable for insulin dependent diabetes mellitus
- Not suitable for diabetic ketoacidosis or diabetic coma
- Hypersensitivity to Glimepiride, sulfonylureas, or sulfonamides
- Severe hepatic dysfunction, severe impairment of renal function, dialysis patients

- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy
- Need for changeover to insulin during pregnancy
- Ingestion with breastfeeding may harm the child
- Necessity of changeover or complete discontinuation of breastfeeding
Precautions & Warnings
- Increased risk of hypoglycaemia in initial weeks of treatment
- Necessity of careful monitoring and dosage adjustment
- Prompt control of hypoglycaemia by immediate intake of carbohydrates
Overdose Effects
- Hypoglycaemic symptoms without loss of consciousness
- Treatment with oral glucose and adjustments in drug dosage or meal patterns
- Severe hypoglycaemic reactions with coma or seizure as medical emergencies
