Indications
- Adjunct to diet and exercise to lower blood glucose in Type II diabetes mellitus
- Use in combination with metformin and insulin
- Control of hyperglycemia
- Increased potential for hypoglycemia
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates insulin release from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Hypoglycemic action persists for 24 hours
Dosage
- Determined by desired blood sugar level
- Initial dose of 1 mg once daily
- Gradual dose titration based on blood sugar monitoring
- Usual dose range of 1 to 4 mg daily
- Single daily dose taken before substantial breakfast or main meal
- Secondary dosage adjustment based on increased sensitivity to insulin

- Not suitable for insulin dependent diabetes mellitus
- Not suitable for diabetic ketoacidosis and diabetic coma

- Not for patients with severe hepatic dysfunction and impaired renal function

Pregnancy & Lactation
- Not to be taken during pregnancy

- Increased risk of hypoglycemia in initial weeks of treatment
- Necessitates careful monitoring
- Prompt control of hypoglycemia with immediate intake of carbohydrates
Overdose Effects
- Hypoglycemia
- Symptoms can be treated with oral glucose and adjustments in drug dosage
- Severe reactions constitute medical emergencies requiring hospitalization
- Treatment with rapid intravenous injection of glucose solution
