Indications
- Adjunct to diet and exercise to lower blood glucose in Type II diabetes mellitus
- May be used concomitantly with metformin for glycaemic control
- Indicated for use in combination with insulin to lower blood glucose
- Potential for increased risk of hypoglycaemia with combined use of Secrin and insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates release of insulin from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin and glucose uptake
Dosage
- Initial dose of 1mg once daily
- Dose titration based on regular blood sugar monitoring
- Usual dose range is 1 to 4mg daily
- Timing and distribution of doses decided by physician based on patient's lifestyle
- Dose adjustment based on changes in weight or lifestyle
- Instructions for changeover from other oral antidiabetics to Glimepiride
Administration
- Tablet must be swallowed with sufficient amount of liquid
Interaction
- Drugs that potentiate hypoglycaemic action of Secrin
- Drugs that attenuate hypoglycaemic action of Secrin
- Potential masking of hypoglycaemic warning symptoms with concomitant beta-receptor blocker, clonidine, guanethidine or reserpine use
- Unpredictable effects of acute and chronic alcohol intake on Secrin activity
Contraindications
- Not suitable for insulin dependent diabetes mellitus
- Not to be used in diabetic ketoacidosis or diabetic coma
- Hypersensitivity to Glimepiride, other sulfonylureas, or other sulfonamides
- Severe hepatic dysfunction, severe impairment of renal function, and dialysis patients

- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy
- Changeover to insulin is necessary during pregnancy
- Harmful effects on breastfeeding child
- Not to be taken by breastfeeding women
Precautions & Warnings
- Initial weeks of treatment may increase risk of hypoglycaemia
- Necessitates careful monitoring
- Prompt control of hypoglycaemia by immediate intake of carbohydrates (glucose or sugar)
Overdose Effects
- Can produce hypoglycaemia
- Mild symptoms treated with oral glucose and adjustments in drug dosage or meal patterns
- Severe reactions require immediate hospitalization and IV glucose solution
- Close monitoring for at least 24 to 48 hours after hypoglycaemic coma
