Indications

  • Adjunct to diet and exercise for Type II diabetes mellitus
  • Use with metformin for inadequate glycaemic control
  • Combination therapy with insulin in uncontrolled hyperglycaemia

Pharmacology

  • Decreases blood glucose concentration
  • Stimulates insulin release from pancreatic beta cells
  • Improves sensitivity of beta cells to glucose stimulus
  • Reduces basal hepatic glucose production
  • Increases peripheral tissue sensitivity to insulin

Dosage

  • Initial dose of 1 mg once daily
  • Dose titration based on regular blood sugar monitoring
  • Usual dose range for well controlled diabetes: 1 to 4 mg daily
  • Single daily dose taken before substantial breakfast or first main meal

Administration

  • Swallow tablet with sufficient amount of liquid

Interaction

  • Potentiating drugs: ACE inhibitors, chloramphenicol, fluconazole
  • Attenuating drugs: Barbiturates, corticosteroids, diazoxide

Contraindications

  • Not suitable for insulin dependent diabetes mellitus
  • Not for diabetic ketoacidosis or coma
  • Hypersensitivity to glimepiride or sulfonylureas
  • Severe hepatic or renal impairment, dialysis patients

Side Effects

  • Hypoglycaemia
  • Temporary visual impairment
  • Nausea, vomiting, diarrhoea, abdominal pain
  • Urticaria
  • Fall in blood pressure

Pregnancy & Lactation

  • Not to be taken during pregnancy
  • Changeover to insulin necessary
  • Harmful if taken during breastfeeding

Precautions & Warnings

  • Increased risk of hypoglycaemia in initial weeks of treatment
  • Careful monitoring and dosage adjustment
  • Prompt control with carbohydrate intake if hypoglycaemia occurs

Overdose Effects

  • Hypoglycaemia
  • Treatment with oral glucose
  • Severe reactions requiring immediate hospitalization
