Indications
- Adjunct to diet and exercise for noninsulin dependent diabetes mellitus (NIDDM)
- May be used concomitantly with metformin
- May be used in combination with insulin
- Combined use of Stimulin and insulin may increase potential for hypoglycaemia
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates release of insulin from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Effects last for 24 hours in non-fasting diabetic patients
Dosage
- Initial dose of 1mg once daily
- Gradual dose titration based on blood sugar monitoring
- Usual dose range in patients with well controlled diabetes is 1 to 4mg daily
- Timing and distribution of doses decided by physician
Administration
- Tablet must be swallowed with sufficient liquid
Interaction
- Interactions with insulin and other oral antidiabetic agents
- Drugs that may potentiate or attenuate the hypoglycaemic action of Stimulin
Contraindications
- Not suitable for insulin dependent diabetes mellitus
- Not suitable for diabetic ketoacidosis or diabetic coma
- Not for patients hypersensitive to Glimepiride or other sulfonylureas
- Not for severe hepatic dysfunction, severe impairment of renal function, or dialysis patients

- Temporary visual impairment
- Nausea
- Vomiting
- Diarrhoea
- Abdominal pain
- Urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy, changeover to insulin necessary
- Not to be taken by breastfeeding women, changeover or discontinuation of breastfeeding necessary
Precautions & Warnings
- Initial weeks of treatment increases risk of hypoglycaemia
- Necessitates careful monitoring and possible adjustment of dosage
- Promptly controlled by immediate intake of carbohydrates
Overdose Effects
- Can produce hypoglycaemia
- Mild symptoms treated with oral glucose and dosage adjustments
- Severe reactions constitute medical emergencies requiring immediate hospitalization
