Indications
- Adjunct to diet and exercise to lower blood glucose in type II diabetes mellitus
- Concomitant use with metformin for inadequate glycaemic control
- Use in combination with insulin for hyperglycaemia
- Increased potential for hypoglycaemia with combined use of Stimulin and insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates insulin release from pancreatic beta cells
- Improves beta cell sensitivity to glucose
- Reduces hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Long-lasting hypoglycaemic action
Dosage
- Dose based on desired blood sugar level
- Initial dose of 1mg once daily
- Gradual dose titration based on regular blood sugar monitoring
- Usual dose range in well-controlled diabetes: 1 to 4mg daily
- Timing and distribution of doses decided by physician
Administration
- Oral administration
- Swallow with sufficient amount of liquid
Interaction
- Potentiates hypoglycaemic action with ACE inhibitors, fluconazole, salicylates, etc.
- Attenuates hypoglycaemic action with barbiturates, corticosteroids, diuretics, etc.
- Potential masking of hypoglycaemic symptoms with beta-blockers, clonidine, etc.
- Inconsistent effects with acute and chronic alcohol intake
Contraindications
- Not suitable for type I diabetes mellitus
- Not for diabetic ketoacidosis or diabetic coma
- Hypersensitivity to Glimepiride or sulfonylureas
- Severe hepatic dysfunction
- Severe impairment of renal function
- Not for dialysis patients

Pregnancy & Lactation
- Not to be taken during pregnancy
- Changeover to insulin necessary for pregnant patients

Precautions & Warnings
- Increased risk of hypoglycaemia in initial weeks of treatment
- Necessitates careful monitoring
- Prompt control with immediate intake of carbohydrates
Overdose Effects
- Can produce hypoglycaemia
- Mild symptoms treated with oral glucose and dosage adjustments
- Severe reactions require immediate hospitalization
- Treatment with intravenous glucose solution
- Close monitoring for at least 24 to 48 hours
