Indications
- Adjunct to diet and exercise for Type II diabetes mellitus
- May be used concomitantly with metformin
- Indicated for use in combination with insulin
- Potential for hypoglycaemia with combined use of Glucorid and insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Decreases blood glucose concentration
- Stimulates release of insulin from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
Dosage
- Individualized based on desired blood sugar level
- Initial dose is 1 mg once daily
- Maintenance dose range is 1 to 4 mg daily
- Timing and distribution of doses decided by physician
Administration
- Tablet must be swallowed with sufficient amount of liquid
Interaction
- Potentiating drugs include ACE inhibitors, aminosalicylic acid, fibrates
- Attenuating drugs include barbiturates, corticosteroids, diuretics
Contraindications
- Not suitable for Type I diabetes mellitus
- Not for diabetic ketoacidosis or diabetic coma
- Hypersensitivity to Glimepiride or sulfonylureas
- Severe hepatic dysfunction or renal impairment
Side Effects
- Hypoglycaemia
- Temporary visual impairment
- Nausea, vomiting, diarrhea, abdominal pain
- Urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy
- Changeover to insulin necessary for pregnancy
- Not to be taken by breastfeeding women
Precautions & Warnings
- Initial weeks require careful monitoring for hypoglycaemia
- Immediate intake of carbohydrates for hypoglycaemia control
Overdose Effects
- May produce hypoglycaemia
- Mild symptoms treated with oral glucose and dosage adjustments
- Severe reactions require immediate hospitalization
