Indications
- Adjunct to diet and exercise for lowering blood glucose in Type II diabetes mellitus
- Use in combination with metformin and insulin for glycaemic control
- Increased risk of hypoglycaemia when used concomitantly with insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates release of insulin from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Hypoglycaemic action persists for 24 hours in non-fasting diabetic patients
Dosage
- Determined by desired blood sugar level
- Lowest effective dose for desired metabolic control
- Initial dose of 1 mg once daily
- Maintenance dose range of 1 to 4 mg daily
- Timing and distribution decided by physician based on patient's lifestyle
- Dose adjustment based on diabetes control and other factors
Administration
- Tablet must be swallowed with sufficient amount of liquid
Interaction
- Potentiating drugs: ACE inhibitors, aminosalicylic acid, anabolic steroids, azapropazone, etc.
- Attenuating drugs: Acetazolamide, barbiturates, calcium channel blockers, etc.
- Concomitant beta-receptor blocker, clonidine, guanethidine, or reserpine may mask hypoglycaemic attack symptoms
- Alcohol intake may potentiate or attenuate activity
Contraindications
- Not suitable for insulin dependent diabetes mellitus
- Not for diabetic ketoacidosis or diabetic coma
- Hypersensitivity to Glimepiride or sulfonylureas
- Severe hepatic dysfunction, severe renal impairment, dialysis patients
Side Effects
- Hypoglycaemia
- Temporary visual impairment
- Nausea, vomiting, diarrhoea, abdominal pain
- Urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy
- Changeover to insulin is necessary
- Harmful when taken during breastfeeding
- Either changeover or discontinuation of breastfeeding is necessary
Precautions & Warnings
- Initial weeks of treatment may increase risk of hypoglycaemia
- Careful monitoring and dosage adjustment necessary
- Prompt control of hypoglycaemia by intake of carbohydrates
Overdose Effects
- Hypoglycaemia
- Requires aggressive treatment with oral glucose
- Severe cases constitute medical emergencies
- Requires hospitalization and continuous monitoring
