Indications
- Adjunct to diet and exercise for lowering blood glucose in noninsulin dependent diabetes mellitus
- Concomitant use with metformin for inadequate glycaemic control
- Use in combination with insulin for lowering blood glucose
- Increased potential for hypoglycaemia with combined use of Senride and insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates release of insulin from pancreatic beta cells
- Improves beta cell sensitivity to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Hypoglycaemic action persists for 24 hours in non-fasting diabetic patients
Dosage
- Governed by desired blood sugar level
- Lowest effective dose for desired metabolic control
- Initial dose: 1 mg once daily
- Gradual dose titration at intervals of 1 to 2 weeks
- Usual dose range in well-controlled diabetes: 1 to 4 mg daily
- Timing and distribution of doses decided by the physician
- Dose reduction with improved diabetes control
- Changeover from other oral antidiabetics with no exact dosage relationship
Administration
Tablet must be swallowed with sufficient amount of liquid
Interaction
- Potentiating drugs: ACE inhibitors, anabolic steroids, coumarin derivatives, salicylates, sulphonamide antibiotics
- Attenuating drugs: Calcium channel blockers, corticosteroids, glucagon, phenytoin, rifampicin, sympathomimetic agents, thyroid hormones
- Masking of hypoglycaemic attack symptoms by beta-receptor blockers, clonidine, guanethidine, reserpine
- Unpredictable interaction with acute and chronic alcohol intake
Contraindications
- Insulin dependent diabetes mellitus
- Diabetic ketoacidosis
- Diabetic coma
- Hypersensitivity to Glimepiride, sulfonylureas, sulfonamides
- Severe hepatic dysfunction
- Severe impairment of renal function
- Dialysis patients

- Fall in blood pressure
Pregnancy & Lactation
- Not suitable during pregnancy
- Changeover to insulin necessary during pregnancy
- Ingestion with breastfeeding may harm the child
- Not suitable for breastfeeding women
Precautions & Warnings
Initial weeks of treatment may increase risk of hypoglycaemia
Overdose Effects
Hypoglycaemic symptoms should be treated with oral glucose
