Indications

Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers basal and postprandial plasma glucose
- Different mechanism of action compared to sulfonylureas
- No hypoglycemia production
- Decreases hepatic glucose production
- Improves insulin sensitivity
- Increases peripheral glucose uptake
Dosage & Administration
- Individualized dosage
- Maximum recommended daily doses
- Initial starting dose for adults (500 mg twice a day or 850 mg once a day)

Interaction
- No chronic co-administration information with Furosemide

Contraindications
- Hypersensitivity to active substance or excipients
- Acute metabolic acidosis
- Severe renal failure
- Acute conditions altering renal function
- Acute or chronic diseases causing tissue hypoxia
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Decrease of vitamin B12 absorption
- Peripheral neuropathy in patients with vitamin B12 deficiency
- Taste disturbance
- Encephalopathy
- Gastrointestinal disorders (nausea, vomiting, diarrhea, abdominal pain)
- Liver function test abnormalities or hepatitis
- Skin reactions (erythema, pruritus, urticaria)
Pregnancy & Lactation
- Uncontrolled diabetes associated risks during pregnancy
- Possible excretion into human breast milk
- No adverse effects observed in breastfed newborns/infants
- Limited data on breastfeeding during metformin treatment
Precautions & Warnings
- Substantial excretion by the kidney
- Risk of accumulation and lactic acidosis
- Lower vitamin B12 level
- Increased risk of hypoglycemia with insulin combination
- Monitoring of renal function
- Discontinuation before surgery and clinical investigations
- Not recommended in conditions causing dehydration, serious infections, or trauma
- Annual estimation of Vitamin B12 levels for continuous therapy patients
Use in Special Populations
- Adjusted dosage based on renal function in elderly subjects
- No effect on growth and puberty detected
- Caution recommended for children aged between 10 and 12 years
- Regular assessment of renal function necessary
- Special caution in situations of potential impaired renal function
Overdose Effects
- No hypoglycemia reported up to 85 gm doses
- Lactic acidosis occurrence
