Comet 850 mg (Tablet)

Indications

• Treatment of type 2 diabetes mellitus
• Management of overweight patients with inadequate glycaemic control

Pharmacology

• Biguanide type oral antihyperglycemic drug
• Lowers basal and postprandial plasma glucose
• Improves insulin sensitivity
• Decreases hepatic glucose production
• Decreases intestinal absorption of glucose

Dosage & Administration

• Dosage must be individualized based on effectiveness and tolerance
• Usual starting dose for adults - 500mg twice a day or 850mg once a day
• Dosage increases should be made in increments of 500mg weekly or 850mg every 2 weeks
• Maximum recommended daily doses for adults - 2000mg-2550mg per day
• Metformin XR dosage, renal impaired patient precautions

Interaction

• Potential interaction with Furosemide and Nifedipine
• Potential interactions with cationic drugs
• Effects on Cimetidine pharmacokinetics
• Drugs that may produce hyperglycemia

Contraindications

• Hypersensitivity to the active substance or to any of the excipients
• Acute metabolic acidosis
• Severe renal failure (GFR <30 mL/min)
• Conditions altering renal function

• Increased risk of hypoglycemia with insulin use
• Regular monitoring of renal function
• Precautions for surgery, serious infections, trauma
• Annual estimation of Vitamin B-12 levels
• Interaction with sulphonylurea, anticoagulants

Use in Special Populations

• Adjustment of metformin dosage in elderly subjects
• No effect on growth and puberty detected
• Caution in prescribing to children aged 10-12 years
• Regular renal function assessment
• Contraindications in patients with GFR <30 ml/min
