Description

  • Comet XR is a biguanide type oral antihyperglycemic drug used in the management of type 2 diabetes.

Dosage Forms

  • Immediate Release Tablet
  • Extended Release Tablet

Indications

  • Treatment of type 2 diabetes mellitus in adults, children from 10 years of age and adolescents

Usability

  • May be used as monotherapy or in combination with other oral antidiabetic agents or with insulin

Pharmacology

  • Lowers both basal and postprandial plasma glucose
  • Decreases hepatic glucose production
  • Decreases intestinal absorption of glucose
  • Improves insulin sensitivity

Dosage Administration

  • Individualized dosage based on effectiveness and tolerance
  • Usual starting dose of 500 mg twice a day or 850 mg once a day for adults
  • Usual starting dose of 500 mg twice a day for children
  • Renal impaired patient precautions

Interaction

  • Possible interactions with Furosemide, Nifedipine, Cationic drugs, Cimetidine, and certain other drugs causing hyperglycemia

Contraindications

  • Hypersensitivity to the active substance or to any of the excipients
  • Acute metabolic acidosis, Severe renal failure, Acute conditions with potential to alter renal function

  • Lower vitamin B12 level
  • Increased risk of hypoglycemia when used in combination with insulin
  • Regular monitoring of renal function advised
  • Usage not advised in conditions causing dehydration or serious infections
  • Potential interaction with anticoagulants

Overdose Effects

  • Hypoglycemia has not been seen with doses up to 85 gm
  • Lactic acidosis is a medical emergency and must be treated in hospital
