Description
Etform is a biguanide type oral antihyperglycemic drug used in the management of type 2 diabetes mellitus. It lowers both basal and postprandial plasma glucose. It decreases hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by an increase in peripheral glucose uptake and utilization.
Indications
- Treatment of type 2 diabetes mellitus in adults
- Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents
- Reduction of diabetic complications in overweight type 2 diabetic adult patients
Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers both basal and postprandial plasma glucose
- Decreases hepatic glucose production
- Decreases intestinal absorption of glucose
- Improves insulin sensitivity
Dosage administration
- Usual starting dose of 500 mg twice a day for adults
- Usual starting dose of 500 mg twice a day for children
- Maximum recommended daily dose of 2550 mg per day for adults
- Usual starting dose of 500 mg once daily for adults (extended release)
- Maximum recommended daily dose of 2000 mg once daily for adults (extended release)
- Not studied in children (extended release)
- Not to be used in patients with eGFR below 30 mL/min/1.73 m^2
Interaction
- Enhances absorption of Nifedipine
- Competes for common renal tubular transport systems with cationic drugs
- No effect on Cimetidine pharmacokinetics
- Certain drugs may produce hyperglycemia and lead to loss of glycemic control
Contraindications
- Hypersensitivity to the active substance or to any of the excipients
- Acute metabolic acidosis
- Severe renal failure
- Conditions with potential to alter renal function
- Conditions causing tissue hypoxia
Side effects
- Hemolytic anemia
- Lactic acidosis
- Decrease of vitamin B12 absorption
- Peripheral neuropathy in patients with vitamin B12 deficiency
- Taste disturbance
- Encephalopathy
- Gastrointestinal disorders like nausea, vomiting, and diarrhea
- Liver function test abnormalities or hepatitis
- Skin reactions such as erythema and pruritus
Pregnancy lactation
- Uncontrolled diabetes during pregnancy increases risk of congenital abnormalities and perinatal mortality
- Metformin is excreted into human breast milk
- Breastfeeding not recommended during metformin treatment
Precautions warnings
- Risk of Etform accumulation and lactic acidosis increases with the degree of impairment of renal function
- Lowers vitamin B12 level
- Increases risk of hypoglycemia when used in combination with insulin or insulin secretagogue
- Regular monitoring of renal function advised
- Discontinue Etform before surgery and clinical investigations
- Stabilization of diabetic patients with Etform and insulin should be carried out in hospital
Use in special populations
- Dosage adjustment based on renal function in elderly subjects
- No effect on growth and puberty detected
- Careful follow-up of the effect on growth and puberty recommended
- Special caution when prescribing to children aged between 10 and 12 years
- Regular assessment of renal function necessary
Overdose effects
- Hypoglycemia not seen with Etform doses up to 85 gm
- Lactic acidosis occurred in such circumstances
- Hemodialysis is the most effective method to remove lactate and Etform
