Description
- Glucomet is an oral antidiabetic medication used in the management of type 2 diabetes mellitus. It lowers both basal and postprandial plasma glucose.

Indication
- Treatment of type 2 diabetes mellitus in adults, children from 10 years of age, and adolescents.
- Reduction of diabetic complications in overweight type 2 diabetic adult patients treated with metformin as first-line therapy after diet failure.
Pharmacology
- Metformin decreases hepatic glucose production.
- Decreases intestinal absorption of glucose.
- Improves insulin sensitivity by increasing peripheral glucose uptake and utilization.
DosageAndAdministration
- Dosage must be individualized based on effectiveness and tolerance, not exceeding maximum recommended daily doses.
- Dosage increases should be made in increments of 500 mg weekly or 850 mg every 2 weeks.
- Metformin XR tablet should be swallowed whole and never crushed, cut, or chewed.
Interaction
- Enhanced absorption of Glucomet by Nifedipine.
- Potential interaction with cationic drugs eliminated by renal tubular secretion.
- Certain drugs may produce hyperglycemia and lead to loss of glycemic control.
Contraindications
- Hypersensitivity to the active substance or any excipients.
- Acute metabolic acidosis, severe renal failure, acute conditions altering renal function, acute or chronic diseases causing tissue hypoxia.
SideEffects
- Hemolytic anemia, lactic acidosis, decrease of vitamin B12 absorption, peripheral neuropathy in patients with vitamin B12 deficiency, taste disturbance, encephalopathy, gastrointestinal disorders, liver function test abnormalities or hepatitis, skin reactions.
PregnancyAndLactation
- Uncontrolled diabetes during pregnancy is associated with increased risk of congenital abnormalities and perinatal mortality.
- Metformin excreted into human breast milk, no adverse effects observed in breastfed newborns/infants.
PrecautionsAndWarnings
- Significant renal excretion, risk of accumulation & lactic acidosis with impaired renal function.
- Lowered vitamin B12 level, increased risk of hypoglycemia when used in combination with insulin or insulin secretagogue.
- Regular monitoring of renal function advised in all diabetics.
- Glucomet therapy should be stopped before certain surgeries and reinstated only after renal function has been regained.

- Dosage adjustment based on renal function in elderly subjects.
- Careful follow-up of effect on growth and puberty in metformin-treated children.
- Particular caution recommended when prescribing to children aged between 10 and 12 years.
OverdoseEffects
- Hypoglycemia not seen with Glucomet doses up to 85 gm.
- Lactic acidosis is a medical emergency and must be treated in hospital.
- Hemodialysis is the most effective method to remove lactate and Glucomet.
