Medicine Details

Indications for use

  • Treatment of type 2 diabetes mellitus in adults
  • Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents
  • Usage as monotherapy for diabetes management
  • Usage in combination with other oral antidiabetic agents or with insulin

Pharmacology

  • Belongs to the biguanide type of oral antihyperglycemic drugs
  • Lowers both basal and postprandial plasma glucose levels
  • Decreases hepatic glucose production
  • Improves insulin sensitivity

Dosage and administration

  • Dosage should be individualized based on effectiveness and tolerance
  • Dosage should not exceed the maximum recommended daily doses
  • Usual starting dose for adults: 500 mg twice a day or 850 mg once a day with meals
  • Usual starting dose for children: 500 mg twice a day with meals
  • Maximum daily dose for adults: 2000 mg, for children: 2000 mg
  • Dosage increases should be made in increments of 500 mg weekly or 850 mg every 2 weeks

Interaction

  • Potential interaction with Furosemide and Nifedipine
  • Possible competition for common renal tubular transport systems with cationic drugs
  • No significant effect on Cimetidine pharmacokinetics
  • Certain drugs may lead to hyperglycemia and loss of glycemic control

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Any type of acute metabolic acidosis
  • Severe renal failure
  • Acute conditions with the potential to alter renal function
  • Acute or chronic diseases that may cause tissue hypoxia

Side effects

  • Possible hematologic, metabolic, neurological, gastrointestinal, hepatic, and dermatologic side effects

Pregnancy and lactation

  • Use of metformin not recommended during pregnancy, insulin is preferred
  • Breastfeeding not recommended due to limited data on adverse effects

Precautions and warnings

  • Substantial excretion by the kidney, risk of accumulation and lactic acidosis
  • Potential decrease in vitamin B12 levels
  • Increased risk of hypoglycemia when used in combination with insulin or insulin secretagogue
  • Regular monitoring of renal function advised

Use in special populations

  • Dosage adjustment for elderly individuals based on renal function
  • Careful follow-up on the effect of metformin on growth and puberty in children
  • Particular caution recommended when prescribing to children aged 10-12 years

Overdose effects

  • No hypoglycemia seen with doses up to 85gm
  • Lactic acidosis is a medical emergency, must be treated in hospital
  • Hemodialysis may be the most effective method for treating lactic acidosis
