Indications
- Treatment of type 2 diabetes mellitus in adults
- Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents
- Reduction of diabetic complications in overweight adults
- Monotherapy for type 2 diabetes mellitus

Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers basal and postprandial plasma glucose
- Different mechanism of action from sulfonylureas
- Does not produce hypoglycemia
- Decreases hepatic glucose production
- Decreases intestinal absorption of glucose
- Improves insulin sensitivity
Dosage & Administration
- Individualized dosage based on effectiveness and tolerance
- Usual starting dose of 500 mg twice a day in adults
- Usual starting dose of 500 mg twice a day in children
- Dosage increases in increments of 500 mg or 850 mg
- Maximum daily doses up to 2550 mg per day for adults
- Dosage adjustment for renal impaired patients

Interaction
- Interaction with Furosemide and Nifedipine
- Potential interaction with cationic drugs
- Minimal effect on Cimetidine pharmacokinetics

Contraindications
- Hypersensitivity to the active substance or excipients
- Acute metabolic acidosis
- Severe renal failure
- Acute conditions altering renal function
- Conditions causing tissue hypoxia

- Decrease of vitamin B12 absorption
- Peripheral neuropathy in patients with vitamin B12 deficiency

Precautions & Warnings
- Substantial excretion by the kidney

Use in Special Populations

Overdose Effects
- Hypoglycemia up to 85 gm doses
