Indications

  • Treatment of type 2 diabetes mellitus
  • Use in adults and children over 10 years old
  • Reduction of diabetic complications in overweight patients
  • Monotherapy or combination therapy with other antidiabetic agents or insulin

Pharmacology

  • Decreases hepatic glucose production
  • Decreases intestinal absorption of glucose
  • Improves insulin sensitivity
  • Lowers both basal and postprandial plasma glucose

Dosage & Administration

  • Individualized dosage based on effectiveness and tolerance
  • Initial dose of 500 mg twice a day for adults
  • Dosage increases made in increments of 500 mg weekly
  • Maximum daily dose of 2550 mg for additional glycemic control
  • Metformin extended release tablet should be swallowed whole

Interaction

  • Enhanced absorption with Nifedipine
  • Potential interaction with cationic drugs eliminated by renal tubular secretion
  • No significant effect on Cimetidine pharmacokinetics
  • Drugs that may produce hyperglycemia and affect glycemic control

Contraindications

  • Hypersensitivity to active substance or excipients
  • Acute metabolic acidosis
  • Severe renal failure (GFR < 30 mL/min)
  • Conditions altering renal function or causing tissue hypoxia

  • Hemolytic anemia
  • Lactic acidosis
  • Decrease of vitamin B12 absorption
  • Peripheral neuropathy
  • Taste disturbance
  • Encephalopathy
  • Gastrointestinal disorders
  • Liver function test abnormalities

Pregnancy & Lactation

  • Increased risk of congenital abnormalities and perinatal mortality with uncontrolled diabetes during pregnancy
  • Excretion into human breast milk
  • Limited data available for breastfeeding during metformin treatment

Precautions & Warnings

  • Substantial renal excretion with increased risk of accumulation and lactic acidosis
  • Lowered vitamin B12 levels
  • Increased risk of hypoglycemia with insulin
  • Regular monitoring of renal function
  • Discontinuation before surgery and reinstatement after renal function is regained
  • Annual estimation of Vitamin B-12 levels for continuous therapy
  • Caution during concomitant therapy with a sulphonylurea

Use in Special Populations

  • Adjustment of metformin dosage in elderly subjects based on renal function
  • Confirmation of type 2 diabetes mellitus before initiating treatment
  • No effect on growth and puberty in controlled clinical studies

Overdose Effects

  • Hypoglycemia not seen with doses up to 85 gm
  • Occurrence of lactic acidosis in overdose situations
  • Treatment of lactic acidosis as a medical emergency
  • Hemodialysis as the most effective method for removing lactate and Glunor
