Indications

  • Treatment of type 2 diabetes mellitus in adults
  • Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents
  • Reduction of diabetic complications in overweight type 2 diabetic adult patients

Pharmacology

  • Management of type 2 diabetes
  • Lowers basal and postprandial plasma glucose
  • Different mechanism of action compared to sulfonylureas
  • Does not produce hypoglycemia
  • Decreases hepatic glucose production
  • Decreases intestinal absorption of glucose
  • Improves insulin sensitivity
  • Increases peripheral glucose uptake and utilization

Dosage & Administration

  • Dosage individualized based on effectiveness and tolerance
  • Usual starting dose for adults: 500 mg twice a day or 850 mg once a day
  • Dosage increases made in increments of 500 mg weekly or 850 mg every 2 weeks
  • Maximum daily dose for adults: 2000 mg per day, given in divided doses
  • Usual starting dose for children: 500 mg twice a day
  • Usual starting dose for Metformin XR in adults: 500 mg once daily with the evening meal
  • Dose should be increased in increments of 500 mg weekly
  • Maximum dose for Metformin XR in adults: 2000 mg once daily with the evening meal or 1000 mg twice daily
  • Metformin extended release tablet should be swallowed whole
  • Special precautions for renal impaired patients

Interaction

  • No information available about interaction with Furosemide
  • Nifedipine may enhance absorption
  • Potential interaction with cationic drugs eliminated by renal tubular secretion
  • No effect on Cimetidine pharmacokinetics
  • Certain drugs may produce hyperglycemia and affect glycemic control

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Acute metabolic acidosis
  • Severe renal failure
  • Conditions altering renal function
  • Conditions causing tissue hypoxia
  • Hepatic insufciency
  • Acute alcohol intoxication
  • Alcoholism

Side Effects

  • Hemolytic anemia
  • Lactic acidosis
  • Decrease of vitamin B12 absorption
  • Peripheral neuropathy in patients with vitamin B12 deficiency
  • Taste disturbance
  • Encephalopathy
  • Gastrointestinal disorders such as nausea, vomiting, diarrhea, and abdominal pain
  • Liver function test abnormalities or hepatitis
  • Skin reactions such as erythema, pruritus, urticaria

Pregnancy & Lactation

  • Uncontrolled diabetes associated with increased risk of congenital abnormalities and perinatal mortality
  • Diabetes during pregnancy should be treated with insulin
  • Metformin excreted into human breast milk
  • Breastfeeding not recommended during metformin treatment

Precautions & Warnings

  • Risk of accumulation and lactic acidosis in patients with impaired renal function
  • Lowering of vitamin B12 levels
  • Increased risk of hypoglycemia when used with insulin or insulin secretagogue
  • Regular monitoring of renal function advised
  • Therapy should be stopped 2-3 days before surgery
  • Glunor not advised in dehydration or serious infections
  • Annual estimation of Vitamin B-12 levels recommended
  • Monitoring of blood glucose during concomitant therapy with a sulphonylurea

Use in Special Populations

  • Adjustment of metformin dosage based on renal function in elderly subjects
  • No detectable effect of metformin on growth and puberty during controlled clinical studies
  • Caution recommended when prescribing to children aged between 10 and 12 years
  • Regular assessment of renal function necessary

Overdose Effects

  • Hypoglycemia not seen with Glunor doses up to 85 g
  • Lactic acidosis a medical emergency, treated in hospital
  • Hemodialysis as the most effective method to remove lactate and Glunor
