Indications
- Treatment of type 2 diabetes mellitus in adults
- Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents
- Reduction of diabetic complications in overweight type 2 diabetic adult patients treated with metformin as first-line therapy after diet failure
Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers basal and postprandial plasma glucose
- Decreases hepatic glucose production
- Improves insulin sensitivity
- Decreases intestinal absorption of glucose
Dosage & Administration
- Usual starting dose of Metformin immediate release tablet for adults: 500 mg twice a day or 850 mg once a day
- Usual starting dose of Metformin immediate release tablet for children: 500 mg twice a day
- Usual starting dose of Metformin extended release tablet for adults: 500 mg once daily with the evening meal
- Usual starting dose of Metformin extended release tablet for children: Not studied
- Do not use Metformin in patients with eGFR below 30 mL/min/1.73 m²
- Swallow Metformin XR tablet whole and never crush, cut or chew
Interaction
- No information available about the interaction of Glutec and Furosemide when co-administered chronically
- Nifedipine appears to enhance the absorption of Glutec
- Glutec had minimal effects on Nifedipine
- Cationic drugs eliminated by renal tubular secretion have the potential for interaction with Glutec
- Certain drugs tend to produce hyperglycemia and may lead to loss of glycemic control
Contraindications
- Hypersensitivity to the active substance or to any of the excipients
- Any type of acute metabolic acidosis
- Severe renal failure (GFR < 30 mL/min)
- Acute conditions with the potential to alter renal function
- Acute or chronic disease that may cause tissue hypoxia
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Taste disturbance
- Encephalopathy
- Gastrointestinal disorders like nausea, vomiting, diarrhea, abdominal pain
- Liver function test abnormalities or hepatitis
- Skin reactions such as erythema, pruritus, urticaria
Pregnancy & Lactation
- Uncontrolled diabetes during pregnancy associated with increased risk of congenital abnormalities and perinatal mortality
- Metformin excreted into human breast milk
- No adverse effects observed in breastfed newborns/infants
- Breastfeeding not recommended during metformin treatment
Precautions & Warnings
- Substantially excreted by the kidney
- May lower vitamin B12 level
- Increases risk of hypoglycemia when used in combination with insulin or insulin secretagogue
- Regular monitoring of renal function advised
- Glutec therapy should be stopped 2-3 days before surgery
- Annual estimation of Vitamin B-12 levels recommended for patients receiving continuous Glutec therapy
Use in Special Populations
- Elderly: Potential for decreased renal function
- Pediatric population: Diagnosis of type 2 diabetes mellitus should be confirmed before treatment with metformin is initiated
- Renal function: Creatinine clearance should be determined before initiating treatment and regularly thereafter
- Glutec contraindicated in patients with GFR < 30 ml/min
Overdose Effects
- Hypoglycemia not seen with Glutec doses up to 85 gm
- Lactic acidosis has occurred in such circumstances
- Lactic acidosis is a medical emergency and must be treated in hospital
- Hemodialysis is the most effective method to remove lactate and Glutec
