Indications
- Treatment of type 2 diabetes mellitus
- Used in overweight patients
- Monotherapy or combination with other oral antidiabetic agents or insulin
- Reduction of diabetic complications
- Recommended for adults and children from 10 years of age
- Pharmacological management of type 2 diabetes
- Lowering of basal and postprandial plasma glucose
- Different mechanism of action compared to sulfonylureas
- Improves insulin sensitivity
- Interferes with hepatic glucose production and intestinal absorption of glucose
Dosage & Administration
- Individualized dosage based on effectiveness and tolerance
- Usual starting dose of 500mg or 850mg
- Dosage increases in increments up to 2000mg per day
- Extended release tablet for once daily dosing
- Dosage adjustments for renal impaired patients
- Swallow whole and do not crush, cut, or chew extended release tablets
Interaction
- Possible interaction with Furosemide and Nifedipine
- Potential competition with cationic drugs for renal tubular transport systems
- Effect on drugs causing hyperglycemia and loss of glycemic control
Contraindications
- Hypersensitivity to active substance or excipients
- Acute metabolic acidosis
- Severe renal failure
- Conditions altering renal function
- Conditions causing tissue hypoxia
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Decrease of vitamin B12 absorption
- Peripheral neuropathy in patients with vitamin B12 deficiency
- Taste disturbance
- Gastrointestinal disorders such as nausea, vomiting, diarrhea
- Liver function test abnormalities or hepatitis
- Skin reactions such as erythema, pruritus, urticaria
Pregnancy & Lactation
- Increased risk of congenital abnormalities and perinatal mortality with uncontrolled diabetes during pregnancy
- Excretion into human breast milk
- No adverse effects observed in breastfed newborns/infants
- Recommendation to use insulin during pregnancy instead of metformin
Precautions & Warnings
- Substantial excretion by the kidney
- Decreased vitamin B12 level
- Increased risk of hypoglycemia with insulin or insulin secretagogue
- Monitoring of renal function and vitamin B12 levels
- Special caution in elderly and pediatric populations
- Regular assessment of renal function
- GFR assessment before treatment initiation
- Temporary discontinuation in the presence of conditions altering renal function
Use in Special Populations
- Adjusted dosage for elderly subjects based on renal function
- Confirmation of type 2 diabetes mellitus before treatment initiation in pediatric population
- No effect on growth and puberty detected in controlled clinical studies
- Special caution when prescribing to children aged between 10 and 12 years
Overdose Effects
- Hypoglycemia not seen with doses up to 85g
- Lactic acidosis a medical emergency
- Treatment of lactic acidosis in hospital
- Removal of lactate and metformin via hemodialysis
