Indications

  • Treatment of type 2 diabetes mellitus in adults
  • Treatment of type 2 diabetes mellitus in children and adolescents
  • Reduction of diabetic complications in overweight patients
  • Monotherapy for type 2 diabetes mellitus
  • Combination therapy with other oral antidiabetic agents
  • Combination therapy with insulin
  • Pharmacological management of type 2 diabetes

Pharmacology

  • Biguanide type oral antihyperglycemic drug
  • Lowers basal and postprandial plasma glucose
  • Different mechanism of action from sulfonylureas
  • Does not produce hypoglycemia
  • Decreases hepatic glucose production
  • Decreases intestinal absorption of glucose
  • Improves insulin sensitivity

Dosage & Administration

  • Individualized dosage based on effectiveness and tolerance
  • Maximum recommended daily doses
  • Starting dose of 500 mg twice a day for adults
  • Starting dose of 850 mg once a day for adults
  • Starting dose of 500 mg twice a day for children
  • Maximum daily dose of 2550 mg for additional glycemic control
  • Dosage increase in increments of 500 mg weekly or 850 mg every 2 weeks
  • Dosage increase made in increments of 500 mg weekly for children
  • Usual starting dose of 500 mg once daily with the evening meal for adults (extended release)
  • Usual starting dose of 500 mg twice a day for children (extended release)
  • Renal impaired patient dosage restrictions
  • Swallow extended release tablet whole

Interaction

  • No chronic co-administration information with Furosemide
  • Enhancement of absorption by Nifedipine
  • Minimal effects on Nifedipine
  • Potential interaction with cationic drugs
  • No effect on Cimetidine pharmacokinetics
  • Drugs that tend to produce hyperglycemia and may lead to loss of glycemic control

Contraindications

  • Hypersensitivity to the active substance or to any of the excipients
  • Acute metabolic acidosis
  • Severe renal failure
  • Acute conditions with potential to alter renal function
  • Acute or chronic diseases causing tissue hypoxia

  • Decreased vitamin B12 absorption
  • Peripheral neuropathy
  • Taste disturbance
  • Encephalopathy

Pregnancy & Lactation

  • Increased risk of congenital abnormalities and perinatal mortality with uncontrolled diabetes during pregnancy
  • Recommendation to use insulin instead of metformin during pregnancy
  • Excretion into human breast milk
  • No adverse effects observed in breastfed newborns/infants
  • Breastfeeding not recommended during metformin treatment

Precautions & Warnings

  • Substantial excretion by the kidney
  • Increased risk of lactic acidosis with impaired renal function
  • Lowering of vitamin B12 levels
  • Increased risk of hypoglycemia when used in combination with insulin
  • Recommendation for regular monitoring of renal function
  • Therapy interruption before surgery and clinical investigations
  • Not advised in conditions causing dehydration or in patients suffering from serious infections or trauma
  • Recommendation for annual estimation of Vitamin B-12 levels
  • Caution with concomitant therapy with a sulphonylurea
  • Reduced renal clearance during cimetidine therapy
  • Potential interaction with anticoagulants

Use in Special Populations

  • Adjustment of metformin dosage based on renal function in elderly subjects
  • Need for regular assessment of renal function in all diabetics
  • Recommendation for confirmation of type 2 diabetes mellitus before treatment in pediatric population
  • Recommended cautious prescribing to children aged between 10 and 12 years
  • Precautions in situations where renal function may become impaired
  • Contraindication in patients with GFR<30 ml/min

Overdose Effects

  • No hypoglycemia seen with doses up to 85 gm
  • Occurrence of lactic acidosis
  • Treatment with hemodialysis for lactic acidosis
