Indications

  • Treatment of type 2 diabetes mellitus
  • Management of type 2 diabetes in overweight patients
  • Monotherapy for adults

Pharmacology

  • Biguanide type oral antihyperglycemic drug
  • Lowers basal and postprandial plasma glucose
  • Decreases hepatic glucose production
  • Improves insulin sensitivity
  • Increases peripheral glucose uptake

Dosage & Administration

  • Individualized dosage based on effectiveness and tolerance
  • Initial dose of 500 mg or 850 mg once or twice daily for adults
  • Children's initial dose of 500 mg twice daily
  • Dose increases in increments of 500 mg or 850 mg
  • Maximum daily doses for adults and children
  • Swallowing extended release tablet whole

Contraindications

Precautions & Warnings

  • Substantial excretion by the kidney
  • Risk of lactic acidosis
  • Lowering of vitamin B12 levels
  • Increased risk of hypoglycemia
  • Regular monitoring of renal function
  • Discontinuation before surgery
  • Annual estimation of Vitamin B-12 levels

Use in Special Populations

  • Adjustment based on renal function in elderly subjects
  • No effect on growth and puberty detected in controlled studies
  • Caution in prescribing to children aged between 10 and 12 years
  • Regular assessment of renal function
  • Caution in situations where renal function may become impaired

Overdose Effects

  • No hypoglycemia seen in doses up to 85 gm
  • Lactic acidosis as a medical emergency
