Dosage Range

  • 500 mg to 2000 mg per day for adults (immediate release tablet)
  • 500 mg to 2000 mg per day for children (immediate release tablet)

  • Not studied in children (extended release tablet)
  • Renal impairment: do not use below eGFR of 30 mL/min/1.73 m^2

Interaction with Other Drugs

  • Furosemide
  • Nifedipine
  • Cationic drugs (e.g., Amiloride, Digoxin, Morphine, Procainamide, Quinidine, Quinine, Triamterene, Trimethoprim, or Vancomycin)
  • Cimetidine
  • Drugs leading to hyperglycemia (e.g., thiazides, diuretics, corticosteroids, phenothiazines, thyroid products, Estrogens, Oral contraceptives, Phenytoin, Nicotinic Acid, sympathomimetics, calcium channel blocking drugs, and Isoniazid)

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Acute metabolic acidosis
  • Severe renal failure
  • Conditions with potential to alter renal function
  • Conditions causing tissue hypoxia
  • Hepatic insufciency
  • Acute alcohol intoxication
  • Alcoholism

Side Effects

  • Hemolytic anemia
  • Lactic acidosis
  • Decreased vitamin B12 absorption
  • Peripheral neuropathy
  • Taste disturbance
  • Encephalopathy
  • Gastrointestinal disorders (e.g., nausea, vomiting, diarrhea, abdominal pain)
  • Hepatitis
  • Skin reactions (e.g., erythema, pruritus, urticaria)

Pregnancy & Lactation

  • Not recommended during pregnancy
  • Excreted into human breast milk
  • Limited data available for breastfeeding

Precautions & Warnings

  • Substantially excreted by the kidney
  • Decreases vitamin B12 level
  • Increases risk of hypoglycemia when used in combination with insulin or insulin secretagogue
  • Regular monitoring of renal function advised
  • Therapy should be stopped before surgery and reinstated only after renal function is regained
  • Not advised in conditions causing dehydration or in patients suffering from serious infections or trauma
  • Reduced renal clearance during cimetidine therapy
  • Possible interaction with anticoagulants

Use in Special Populations

  • Elderly: dosage adjustment based on renal function
  • Pediatric population: diagnosis confirmation required before treatment
  • No effect on growth and puberty detected during controlled clinical studies
  • Particular caution recommended for children aged between 10 and 12 years
  • Renal function assessment before treatment initiation and regularly thereafter

Overdose Effects

  • Hypoglycemia not seen up to 85 gm
  • Lactic acidosis a medical emergency
  • Treatment with hemodialysis
