Indications

  • Treatment of type 2 diabetes mellitus
  • Management of type 2 diabetes in overweight patients
  • Monotherapy for adults with diabetes
  • Combination therapy for adults with diabetes
  • Use in children from 10 years of age and adolescents

Pharmacology

  • Biguanide type oral antihyperglycemic drug
  • Lowers both basal and postprandial plasma glucose
  • Decreases hepatic glucose production
  • Decreases intestinal absorption of glucose
  • Improves insulin sensitivity

Dosage & Administration

  • Individualized dosage based on effectiveness and tolerance
  • Usual starting dose of 500mg - 850mg
  • Maximum daily dose of 2000mg - 2550mg
  • Extended release tablet dosage instructions
  • Renal impaired patient dosage restrictions

Interaction

  • No chronic interaction with Furosemide
  • Enhanced absorption with Nifedipine
  • Potential interaction with cationic drugs
  • No effect on Cimetidine pharmacokinetics
  • Certain drugs that produce hyperglycemia

Contraindications

  • Hypersensitivity to active substance or excipients
  • Acute metabolic acidosis
  • Severe renal failure (GFR < 30 mL/min)
  • Conditions altering renal function
  • Conditions causing tissue hypoxia

Side Effects

  • Hemolytic anemia
  • Lactic acidosis
  • Decrease in vitamin B12 absorption
  • Taste disturbance
  • Gastrointestinal disorders
  • Skin reactions

Pregnancy & Lactation

  • Increased risk of congenital abnormalities
  • No observed adverse effects in breastfed newborns/infants
  • Excretion into human breast milk
  • Breastfeeding not recommended during treatment

Precautions & Warnings

  • Substantially excreted by the kidney
  • Risk of Meforin accumulation and lactic acidosis
  • Lowering of vitamin B12 level
  • Increased risk of hypoglycemia
  • Regular monitoring of renal function

Use in Special Populations

  • Adjusted dosage for elderly subjects
  • No effect on growth and puberty detected
  • Caution advised for children aged between 10 and 12 years
  • Renal function assessment before and during treatment

Overdose Effects

  • Hypoglycemia not seen with doses up to 85g
  • Lactic acidosis observed in overdose
  • Treatment of lactic acidosis in hospital
