Description
- Meforin tablet is used for the treatment of type 2 diabetes mellitus in adults and children from 10 years of age and adolescents.
Indications
- Treatment of type 2 diabetes mellitus in overweight patients
- Reduction of diabetic complications in overweight type 2 diabetic adult patients treated with metformin as first-line therapy after diet failure
Mode of Action
- Biguanide type oral antihyperglycemic drug
- Lowers both basal and postprandial plasma glucose
- Decreases hepatic glucose production and intestinal absorption of glucose
- Improves insulin sensitivity by increasing peripheral glucose uptake and utilization
Dosage and Administration
- Available as immediate release and extended release tablets
- Must be individualized based on effectiveness and tolerance
- Usual starting dose for adults is 500 mg twice a day or 850 mg once a day, given with meals
- Maximum recommended daily doses for adults is 2000 mg per day for immediate release and 2000 mg once daily for extended release
Interaction
- Potential interaction with Furosemide and Nifedipine
- Potential interaction with cationic drugs and certain other medications that tend to produce hyperglycemia
Contraindications
- Hypersensitivity to the active substance or any excipients
- Acute metabolic acidosis
- Severe renal failure (GFR < 30 mL/min)
- Acute conditions or diseases that may alter renal or tissue function
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Taste disturbance
- Gastrointestinal disorders like nausea, vomiting, and diarrhea
- Liver function test abnormalities or hepatitis
- Skin reactions like erythema, pruritus, and urticaria
Pregnancy and Lactation
- Not recommended for use during pregnancy, insulin is preferred
- Excreted into human breast milk, breastfeeding not recommended during treatment
Precautions and Warnings
- Substantially excreted by the kidney, requires regular monitoring of renal function
- May lower vitamin B12 levels
- Increased risk of hypoglycemia when used in combination with insulin or insulin secretagogue
Use in Special Populations
- Dosage adjustment in elderly subjects based on renal function
- Confirmation of type 2 diabetes mellitus before treatment initiation in pediatric population
- Recommended caution when prescribing to children aged between 10 and 12 years
Overdose Effects
- Hypoglycemia not observed up to 85 gm doses
- Lactic acidosis is a medical emergency and must be treated in hospital
- Hemodialysis is the most effective method for removal
