Indications
- Treatment of type 2 diabetes mellitus

Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers basal and postprandial plasma glucose
- Decreases hepatic glucose production
- Improves insulin sensitivity
- Decreases intestinal absorption of glucose
Dosage & Administration
- Dosage individualized based on effectiveness and tolerance
- Usual starting dose for adults - 500 mg twice a day or 850 mg once a day
- Usual starting dose for children - 500 mg twice a day
- Maximum recommended daily dose - 2000 mg for adults and children
- Usual starting dose for extended release tablet - 500 mg once daily

Interaction
- Possible interaction with Furosemide and Nifedipine
- Potential interaction with cationic drugs and drug-induced hyperglycemia
- No effect on Cimetidine pharmacokinetics
Contraindications
- Hypersensitivity to the active substance
- Severe renal failure
- Acute metabolic acidosis
- Acute conditions altering renal function
- Conditions causing tissue hypoxia

- Vitamin B12 absorption decrease
- Neuropathy in patients with vitamin B12 deficiency

Pregnancy & Lactation
- Uncontrolled diabetes risk during pregnancy
- Excretion into human breast milk

Precautions & Warnings
- Substantial excretion by the kidney
- Risk of lactic acidosis and Meformin accumulation
- Lowering of vitamin B12 level
- Increased risk of hypoglycemia

Use in Special Populations
- Elderly dosage adjustment based on renal function
- No effect on growth and puberty detected in controlled clinical studies
- Caution recommended for children aged between 10 and 12 years

Overdose Effects
- No hypoglycemia observed up to 85 gm doses
- Lactic acidosis as a potential outcome
- Hemodialysis as the most effective method for removal
