Indications
- Treatment of type 2 diabetes mellitus

Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers basal and postprandial plasma glucose
- Different mechanism of action from sulfonylureas
- Does not produce hypoglycemia
- Decreases hepatic glucose production
- Improves insulin sensitivity
Dosage & Administration
- Dosage must be individualized for effectiveness and tolerance
- Maximum recommended daily doses should not be exceeded
- Dosage increases should be made in increments
- Different dosages for adults and children
- Extended release tablet should not be crushed or chewed
- Dosage adjustment for renal impaired patients
Interaction
- Potential interaction with Furosemide

- Liver function test abnormalities or hepatitis
- Skin reactions
Pregnancy & Lactation
- Uncontrolled diabetes associated with increased risk of congenital abnormalities and perinatal mortality
- Recommendation to use insulin during pregnancy
- Excretion into human breast milk
- Limited data available for breastfeeding
Precautions & Warnings
- Substantial excretion by the kidney
- Increased risk of lactic acidosis
- Lowering of vitamin B12 levels
- Increased risk of hypoglycemia
- Regular monitoring of renal function advised
- Discontinuation before surgery and reinstatement after renal function regains
- Caution in conditions causing dehydration or serious infections
- Annual estimation of Vitamin B-12 levels for continuous therapy
- Special precautions during concomitant therapy with sulphonylurea
- Possible reduced renal clearance during cimetidine therapy
- Potential interaction with anticoagulants
Use in Special Populations
- Adjustment of dosage based on renal function in elderly subjects
- No effect on growth and puberty detected in controlled clinical studies
- Careful follow-up of effect on growth and puberty recommended
- Particular caution recommended for children aged between 10 and 12 years
- Regular assessment of renal function
- Decreased renal function in elderly subjects
- Special caution in situations where renal function may become impaired
- Contraindicated in patients with GFR<30 ml/min
Overdose Effects
- Hypoglycemia not seen with doses up to 85 gm
- Lactic acidosis occurred in such circumstances
- Lactic acidosis is a medical emergency
