Indications

  • Treatment of type 2 diabetes mellitus
  • Used as monotherapy or in combination with other oral antidiabetic agents or with insulin
  • Reduction of diabetic complications in overweight patients

Pharmacology

  • Biguanide type oral antihyperglycemic drug
  • Lowers both basal and postprandial plasma glucose
  • Improves insulin sensitivity
  • Decreases hepatic glucose production

Dosage & Administration

  • Dosage must be individualized based on effectiveness and tolerance
  • Usual starting dose for adults is 500 mg twice a day
  • Usual starting dose for children is 500 mg twice a day
  • Maximum daily dose is 2000 mg, given in divided doses

Interaction

  • Enhancement of absorption by Nifedipine
  • Potential interaction with cationic drugs
  • Certain drugs tend to produce hyperglycemia and may lead to loss of glycemic control

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Acute metabolic acidosis
  • Severe renal failure
  • Acute conditions altering renal function
  • Acute or chronic diseases causing tissue hypoxia

  • Hemolytic anemia
  • Lactic acidosis
  • Decrease of vitamin B12 absorption
  • Taste disturbance
  • Encephalopathy
  • Gastrointestinal disorders
  • Liver function test abnormalities
  • Skin reactions

Pregnancy & Lactation

  • Increased risk of congenital abnormalities and perinatal mortality
  • Not recommended during metformin treatment
  • Excreted into human breast milk

Precautions & Warnings

  • Substantially excreted by the kidney
  • Lowering of vitamin B12 level
  • Regular monitoring of renal function advised
  • Annual estimation of Vitamin B-12 levels recommended
  • Interaction with anticoagulants is a possibility

Use in Special Populations

  • Dosage adjustment in elderly subjects based on renal function
  • No effect on growth and puberty detected
  • Particular caution in prescribing to children aged between 10 and 12 years
  • Creatinine clearance determination before initiating treatment
  • Metomin is contraindicated in patients with GFR < 30 ml/min

Overdose Effects

  • Lactic acidosis is a medical emergency
  • Most effective method to remove lactate and Metomin is hemodialysis
