Indications
- Treatment of type 2 diabetes mellitus in adults
- Treatment of type 2 diabetes mellitus in children and adolescents
Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers both basal and postprandial plasma glucose
- Improves insulin sensitivity
- Decreases hepatic glucose production
- Decreases intestinal absorption of glucose
Dosage & Administration
- Dosage must be individualized based on effectiveness and tolerance
- Usual starting dose for adults: 500 mg twice a day or 850 mg once a day
- Maximum daily dose for adults: 2000-2550 mg per day
- Usual starting dose for children: 500 mg twice a day
- Maximum daily dose for children: 2000 mg per day
- Dosage for renal impaired patients
Interaction
- No significant interaction with Furosemide
- Enhances absorption of Nifedipine
- Potential interaction with cationic drugs
- Minimal effects on Cimetidine pharmacokinetics
- Certain drugs may lead to loss of glycemic control
Contraindications
- Hypersensitivity to active substance or excipients
- Acute metabolic acidosis
- Severe renal failure
- Conditions altering renal function
- Conditions causing tissue hypoxia

Pregnancy & Lactation

Precautions & Warnings
- Substantially excreted by the kidney
- Risk of hypoglycemia in combination with insulin
- Regular monitoring of renal function advised
- Therapy should be stopped before surgery
- Potential interaction with anticoagulants
Use in Special Populations
- Elderly: Dosage should be adjusted based on renal function
- Pediatric population: Diagnosis of type 2 diabetes mellitus should be confirmed before treatment
- Renal function: Creatinine clearance should be determined before initiating treatment and regularly thereafter
Overdose Effects
- Hypoglycemia not seen with standard doses
- Lactic acidosis is a medical emergency and must be treated in hospital
- Hemodialysis for removing lactate and Metsa
