Indications
- Treatment of type 2 diabetes mellitus
- Management of type 2 diabetes in overweight patients
- Monotherapy for adults with type 2 diabetes
- Combination therapy for adults with type 2 diabetes
- Combination therapy for children from 10 years of age and adolescents
Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers basal and postprandial plasma glucose
- Different mechanism of action than sulfonylureas
- Does not produce hypoglycemia
- Decreases hepatic glucose production
- Decreases intestinal absorption of glucose
- Improves insulin sensitivity
Dosage & Administration
- Individualized dosage based on effectiveness and tolerance
- Usual starting dose of 500 mg twice a day for adults
- Usual starting dose of 500 mg twice a day for children
- Dosage increases in increments of 500 mg or 850 mg
- Maximum daily dose of 2000 mg for adults
- Maximum daily dose of 2000 mg for children
- Swallow extended release tablet whole
- Dosage adjustments for renal impaired patients
Interaction
- No chronic interaction with Furosemide
- Enhanced absorption with Nifedipine
- Potential for interaction with cationic drugs
- No effect on Cimetidine pharmacokinetics
- Certain drugs may produce hyperglycemia
Contraindications
- Hypersensitivity to active substance or excipients
- Acute metabolic acidosis
- Severe renal failure
- Acute conditions altering renal function
- Conditions causing tissue hypoxia
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Decrease of vitamin B12 absorption
- Peripheral neuropathy in patients with vitamin B12 deficiency
- Taste disturbance
- Encephalopathy
- Gastrointestinal disorders
- Liver function test abnormalities or hepatitis
- Skin reactions
Pregnancy & Lactation
- Increased risk of congenital abnormalities and perinatal mortality
- Metformin excreted into human breast milk
- No adverse effects observed in breastfed newborns/infants
- Breastfeeding not recommended during metformin treatment
Precautions & Warnings
- Substantially excreted by the kidney
- Decreased renal function in elderly subjects
- Lower vitamin B12 level
- Increased risk of hypoglycemia in combination therapy
- Monitoring of renal function advised
- Interruption of therapy before certain clinical investigations
- Monitoring of Vitamin B12 levels during continuous therapy
- Hypoglycemia risk with combined therapy
Use in Special Populations
- Adjusted dosage for elderly subjects
- Diagnosis confirmation before treatment initiation in children
- No effect on growth and puberty detected in controlled studies
- Regular assessment of renal function in diabetics
- Caution when prescribing to children aged 10-12 years
Overdose Effects
- No hypoglycemia seen with doses up to 85 gm
- Lactic acidosis occurred in overdose circumstances
- Treatment with hemodialysis may be necessary
